Geel's housing and support services combine a coordinated, self-directed approach that is individually tailored and trauma informed. Our model promotes increased self-sufficiency, self-worth, recovery, and the ability for those we serve to live independently within their communities. Our philosophy: we believe that people with mental illness thrive when they take increasing responsibility for themselves and become more productive in their communities. THE MAJOR GOAL AREAS ARE: REHABILITATION, MENTAL HEALTH, PHYSICAL HEALTH AND SOCIALIZATION AND THE OBJECTIVES MAY INCLUDE: BUDGETING, MEDICATION MANAGEMENT, CRISIS INTERVENTION, PREVOCATIONAL TRAINING, ENTITLEMENTS, SUPPORTIVE COUNSELING, GROUP DYNAMICS AND REFERRAL TO APPROPRIATE SERVICE PROVIDERS. THE MOST SIGNIFICANT ACCOMPLISHMENT FOR THIS PROJECT DURING THE OPERATING YEAR WAS IN THE AREA OF RESIDENTIAL HOUSING STABILITY. THE PROJECT MAINTAINED AN AVERAGE OCCUPANCY RATE OF 99.6%.THIS IS A SIGNIFICANT ACCOMPLISHMENT GIVEN THE FACT THE ALMOST 47% OF THE RESIDENTS OF THIS PROGRAM HAVE BEEN IDENTIFIED AS CHRONICALLY HOMELESS. TO ACHIEVE SUCH A HIGH RATE OF OCCUPANCY IS A TESTAMENT TO THE COLLABORATION OF STAFF AND RESIDENTS AT THIS PROJECT. THE PROJECT HAS EMPHASIZED A WELLNESS SELF-MANAGEMENT MODEL THAT INCLUDES PERSONALIZED RECOVERY TO THE RESIDENTS. PRIMARILY, WELLNESS SELF-MANAGEMENT IS ENCOURAGED THROUGH GROUP AND INDIVIDUAL CASE MANAGEMENT. THE NUMEROUS GROUPS AND WORKSHOPS THAT ARE OFFERED IN HOUSE TO THE RESIDENTS OF THIS PROJECT INCLUDE; HEALTH AND WELLNESS, SUBSTANCE ABUSE, ACTIVITIES OF DAILY LIVING (ADL), CAREER AND EDUCATION, COMPUTER TRAINING, ART THERAPY, WOMENS, LATINO, AND SOCIALIZATION. IN THE HEALTH AND WELLNESS GROUPS, SUCH TOPICS SUCH AS; DIABETES MANAGEMENT, HYPERTENSION, HIV/AIDS, VACCINES AND IMMUNIZATIONS, HEALTHY NUTRITION, HEPATITIS C, HEALTH & FITNESS, SMOKING CESSATION, ORAL HEALTH, SUGARY DRINKS AND AN ANNUAL HEALTH FAIR ARE COVERED. ADDITIONALLY, ALL RESIDENTS PARTICIPATING IN GROUPS AND INDIVIDUAL SESSIONS ARE ENCOURAGED TO ATTEND ALL BEHAVIORAL AND MEDICAL APPOINTMENTS, HAVE ANNUAL PHYSICAL EXAMS, AND IMPROVE NUTRITIONAL HABITS AND MAKE LIFESTYLE CHANGES. |
