It is the responsibility of all board directors to familiarize themselves and comply with the policy. In addition to the appropriate disclosures required by the policy, each board member is annually required to read the policy and to sign a conflict of interest form. Copies of all completed forms are maintained in the organization's books and records in the executive office.”
