Governance Explanations

Form 990, Part VI, Section B, line 11B

A copy of this Form 990 was provided to the company's Board of Directors (the Governing Body) who reviewed and approved the form prior to its filing with the Internal Revenue Service.

Form 990, Part VI, Section B, line 12C

The conflict of interest questionnaire is completed annually at a minimum or whenever a potential conflict of interest arises. It is completed by all members of the board of directors and all administrative level staff including middle managers, department directors and assistant directors. The executive director/chief executive officer submits a quarterly report to an oversight committee of any staff whose relatives are hired. Approval of the oversight committee is required in advance for any related hiring that will result in compensation of more than $5,000. Any person with a potential conflict is prohibited from participating in the deliberation. Four board members have been appointed to the oversight committee.

Form 990, Part VI, Section B, line 15

A member of the board of directors finance committee collects and reviews compensation information of other organizations similar to visions. After comparing information, it is discussed with the rest of the members of the committee. The process is done each year in september for the upcoming fiscal year.

Form 990, Part VI, Section C, line 19

Audited financial statements are available at visions website. Governing documents and conflict of interest policy are available upon request. Annual report is mailed to all donors of $100 or more.

VISIONS Center on Aging and Intergenerational Program: One of only two citywide Innovative Senior Centers in New York City, dedicated to serving special populations. First ever publicly funded New York City senior center for elders with vision loss enrolled 714 seniors and met all the outcomes required. The Intergenerational Program is a program offered through VISIONS Center on Aging where homebound seniors with vision loss receive home visits once every other week from sighted high school students. VISIONS Intergenerational Program was named a Program of Distinction by Generations United. Pre-College Program: The residential college preparatory program, launched in July of 2013. The program was designed for legally blind high school juniors and seniors. The goal of the program is to help young people prepare for college life. The program was held on the campus of Manhattanville College in Purchase, NY. The program served 23 legally blind students from the New York area during the summer. SUMMER TRANSITION PROGRAM AT QUEENS COLLEGE: The Summer Transition Program is a four-week long Sunday night to Friday afternoon residential program for transition age youth (typically 14 -16 years old). The new program at Queens College is designed to build students' skills for independence, college and employment in a setting away from their homes. Students work to develop their skills for daily living as well as college readiness. The program includes use of technology, orientation & mobility instruction, social interaction, fitness and recreation on the Queens College campus as well as trips around New York City. Self- Advocacy is promoted throughout the program. The program served 20 participants. MEDICAID FACILITATED ENROLLER PROGRAM: VISIONS ended the Medicaid Facilitated Enroller Program and expanded the role to Facilitated Enrollment in all benefit programS. VISIONS received a grant for the NYC Youth and Vision Loss Coalition, a coalition of parents, educators, service providers, consumers, organizations, agencies, employers and youth to assist and represent children with vision loss, including those with multiple disabilities and their parents/caregivers residing in New York City. The Coalition membership includes over 30 organizations.
