Hope For Youth is dedicated to facilitating positive change in the lives of children,adolescents and families. We provide stable,nurturing,residential care,foster care,preventive and out-patient programs which educate,motivate and empower individuals to become self-sufficient and achieve their full potential.

The Form 990 is prepared by an independent third party with the assistance of the CFO. The draft Form 990 is presented to the Audit Committee for review. The Audit Committee reviews and approves the draft Form 990 for filing. The final 990 is presented to the remainder of the Board members for review.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Each board member, officer and management staff are required to annually review and certify that they have read the policies and agree to comply with the policy. The Governance committee is responsible for monitoring the annual policy disclosure process.

The Personnel and Finance Committee reviews external comparable market data from associations in the industry and reviews Executive Director, Deputy Executive Director and CFO's salaries. Form 990, Part III, Line 4D: Other Program Services Description

OTHER PROGRAM SERVICES 4: Suffolk County Community Reinvestment Program - Through the Suffolk County Community Reinvestment Program (CRP), Hope For Youth strives to teach at-risk youth healthy behaviors to avoid detention, legal involvement and violence, while improving school attendance. OTHER PROGRAM SERVICES 5: Out-Patient Chemical Dependency Treatment - The Hope for Youth Family Service Center is licensed by the New York State Office of Alcoholism and Substance Abuse Services (OASAS) as an Out-Patient Adolescent Chemical Dependency Program. The target population includes adolescents who are using drugs and alcohol or children who come from family systems which have a history of substance abuse. The Family Service Center provides clinical services designed specifically to address the issues unique to adolescents, children and families struggling with substance abuse. OTHER PROGRAM SERVICES 6: Suffolk Intensive Case Management - Hope For Youth is committed to working with families who have severe odds against them, and ICM has an impressive success rate of 75 to 80 percent for keeping families together. The Suffolk ICM Program covers all of Suffolk County. It is capable of meeting the needs of up to 24 children/families at any one time. OTHER PROGRAM SERVICES 7: Multi-Dimensional Family Therapy Program - Hope For Youth Out-Patient Chemical Dependence Clinic began the Multi-Dimensional Family Therapy (MDFT) Program as an addition to the Out-Patient Chemical Dependency for Youth clinic. Both the clinic and the MDFT program specialize in the treatment of adolescent substance abuse. OTHER PROGRAM SERVICES 8: School Based Prevention Program school based drug prevention program funded by OASAS. OTHER PROGRAM SERVICES 9: Runaway and Homeless Shelter - the HFY Shelter provides a safe, supportive and secure environment for all Suffolk County youth between the ages of 12 and 20 who find themselves in crisis and need a haven. The program provides support services to all residents 24 hours per day, 7 days a week and 365 days a year for up to 30 days. While youth are with us they will be provided with safe shelter, case management services, individual and family counseling, transportation, food, clothing, educational assistance and advocacy, medical care coordination and referrals, living skills development and other essential services. Hope For Youth can and will meet all the basic needs of this population while providing each youth with the support they need to reach their goals. OTHER PROGRAM SERVICES 10: Seaford Group Home - CONSISTS OF ONE COMMUNITY BASED GROUP HOME LOCATED IN NASSAU COUNTY. ADOLESCENT MALES BETWEEN THE AGES OF 13 AND 21 ARE REFERRED BY THE OFFICE OF CHILDREN AND FAMILY SERVICES, AND BOTH NASSAU AND SUFFOLK COUNTY DEPARTMENTS OF SOCIAL SERVICES AND PROBATION. HFY WORKS CLOSELY WITH THE REFERRAL SOURCE AND EACH CHILD AND FAMILY IN ORDER TO IDENTIFY THE STRENGTH AND NEEDS OF EACH FAMILY AND TO DESIGN A TREATMENT PLAN THAT WILL ULTIMATELY LEAD TO THE REUNIFICATION OF THE FAMILY OR A SUCCESSFUL DISCHARGE OF AN OLDER ADOLESCENT TO INDEPENDENT LIVING

The Office of Mental Health also utilizes this program for respite placements. Intakes are accepted 24 hours per day; placements are usually short term up to 90 days. Youth entering this program are unable to maintain appropriate levels of social interaction, lacking the ability to function without presenting a threat to the safety of themselves and others. Children attend a full school day on site at HFY with instruction by a NY State certified school teacher.

HFY WORKS CLOSELY WITH THE REFERRAL SOURCE AND EACH CHILD AND FAMILY IN ORDER TO IDENTIFYTHE STRENGTH AND NEEDS OF EACH FAMILY AND TO DESIGN A TREATMENT PLAN THAT WILL ULTIMATELY LEAD TO THE REUNIFICATION OF THE FAMILY OR A SUCCESSFUL DISCHARGE OF AN OLDER ADOLESCENT TO INDEPENDENT LIVINGTHE OTHER HTP PROGRAM IS A COED SIBLING REUNIFICATION GROUP HOME. THIS PROGRAM IS DESIGNED TO BRING LARGE SIBLING GROUPS TOGETHER THAT HAVE BEEN PLACED IN SEPARATE FOSTER CARE SETTINGS IN ORDER TO ALLOW THEM TO RESIDE TOGETHER IN A SINGLE HOME ENVIRONMENT AND ASSIST WITH OVERALL FAMILY REUNIFICATION AND PERMANENCY PLANNING. THERAPEUTIC SERVICES ARE BUILT IN AS WELL IN ORDER TO ASSIST THE FAMILY WITH WORKING TOWARDS REUNIFICATION.
