Mission and Program Overview

Mission

Community health center (chc) is non-profit agency providing access to and encouraging participation in comprehensive primary health care and social services. (see schedule o)its quality services are available to all, and particularly to those who cannot gain access to such services elsewhere. Chc takes leadership in promoting interagency cooperation. It is based on consumer control and is committed to ensuring human rights and respecting human dignity; as such, it strives to be a voice and vehicle for social change.

Medical, dental, mental health, and social services: the medical component provides primary health care and related services including prenatal care. The dental component provides a full range of denistry. The mental health component provides child guidance services, adult therapy and counseling. The domestice violence component provides counseling and shelter for battered women and their children.

Community health center, inc. Engaged psl consulting, a not-for-profit compensation consulting firm,in 2010 to conduct an independent survey of ceo compensation at comparable organizations. The consulting firm contacted several organizations, sent interview guides, and gathered compensation through phone interviews with survey organization principals. The information obtained through these interviews was supplemented with form 990 information from other identified comparable organizations. After this review was complete, the board discussed and approved the compensation.

The organization has received exemption from federal income tax under section 501(c)(3) of the internal revenue code. The organization has also been classified as an entity that is not a private foundation within the meaning of section 509(a) and qualifies for deductible contributions as provided in section 170(b)(1)(a)(vi). Management has reviewed the organization's reporting and believe they have not taken tax positions that are more likely than not to be determined to be incorrect by the internal revenue service and therefore, no adjustments or disclosures are required. The organization is subject to routine audits by taxing jurisdictions; however there are currently no audits for any tax periods pending or in progress.

IT PROVIDES CARE IN TRADITIONAL OFFICE SETTINGS AS WELL AS IN SCHOOL BASED HEALTH CENTERS, HOMELESS SHELTERS, AND DOMESTIC VIOLENCE CENTERS. CHC'S MODEL OF CARE IS BASED ON THE PRINCIPLES OF ADVANCED ACCESS, PLANNED CARE, AND THE CHRONIC CARE MODEL SUPPORTED BY ELECTRONIC MEDICAL RECORDS AND A TEAM-BASED APPROACH TO CARE. CHC ALSO PROVIDES PRENATAL CARE AND SPECIFIC SPECIALTY AND SUPPORT SERVICES INCLUDING HIV/AIDS, PODIATRY, DIABETES EDUCATION, NUTRITION, AND PHARMACY CONSULTATION. CHC IS ACTIVELY INVOLVED IN TRAINING THE NEXT GENERATION OF MEDICAL (SEE SCHEDULE O) HEALTHCARE PROFESSIONALS AND WELCOMES STUDENTS AND RESIDENTS FROM MULTIPLE UNIVERSITIES AND COLLEGES OF NURSING, MEDICINE, AND PHARMACY AS WELL AS OPERATING THE COUNTRY'S FIRST RESIDENCY PROGRAM FOR FAMILY NURSE PRACTITIONERS.
