Clinical research in cardiovascular disease is another major component of the program.At the completion of FY20, the Division of Cardiology was comprised of 33 full time faculty cardiologists. Cardiology division members published 46 manuscripts in peer-reviewed journals in 2020. Many of these articles were published in top medical journals. There was a significant decrease in volume as elective cases were cancelled and some patients delayed care for urgent cardiovascular conditions. In addition, one of the Catheterization labs had to be outfitted with special barriers to care for patients with COVID-19. Specific protocols were quickly developed to care for these patients who often had unusual features associated with their cardiac presentations. Total cases performed in the Cardiac Catheterization laboratory in FY20 was 4,398 representing an 8.7% decrease compared to FY19 when 4,817 cases were performed. The number of coronary cases was 4,211 representing a 9.1% decrease compared to the prior year. The Percutaneous Coronary Intervention (PCI) volume decreased from 1,262 to 1,180 representing a 6.5% decrease in the number of cases. There was a decrease in the number of PCI cases involving chronic total occlusions (33 vs. 53). There was a decrease in the post procedure PCI length of stay from 2.2 days to 2 days. The percentage of PCI cases performed from the radial artery remains high at 70%. This compares to the national average of approximately 50%. The number of peripheral vascular cases performed in the Cardiac Catheterization laboratory decreased from 70 to 29 representing a 58.6% decline.Structural Heart Disease:Hartford Hospital structural heart physicians performed a total of 280 Transcatheter Heart Valve System (TAVR) in FY20 representing a 12.5% decrease compared to procedures performed in FY19. This decrease in procedural volume was limited to the months of March, April and May 2020 and has been attributed to the COVID19 pandemic. To date, over 1,600 TAVR procedures have been performed since 2012 with both balloon-expandable and self-expanding bioprostheses, utilizing transfemoral, subclavian, carotid, direct aortic and transapical approaches. Since 2012, the Hartford Hospital Multidisciplinary Valve committee has met weekly to evaluate and triage potential TAVR candidates. Well over 2000 patients have been evaluated to-date by the highly experienced team of interventional cardiologists, cardiac surgeons, echocardiographers, CT imaging specialists and valve coordinator APRN's. Structural Heart Governance meetings have also been effectively utilized to critically examine patient outcomes, initiate quality improvement projects and support the ongoing evolution of procedural technique.
