“The draft form 990 is reviewed and approved by the executive committee. The executive committee communicates the availablility of the draft form 990 to the full board for their review and comment within 15 days. Comments are addressed by the executive committee if necessary and provides the final approval and issuance of the form 990.”

“Directors, officers and key employees of the cemetery are required to sign the conflict of interest policy at the time of appointment or hire. Thereafter, each director, officer and key employee shall annually sign a statement which affirms such person: - has received a copy of the conflict of interest policy, - has read and understands the policy, - has agreed to comply with the policy, and - understands the cemetery is charitable and in order to maintain its federal tax exemption it must engage primarily in activities which accomplish one or more of its tax-exempt purposes.”

“Members of the executive committee review all executive employees compensation annually. Performance evaluation, review of comparable salary data, and budget constraints are given consideration when establishing an executive's compensation. Any person with a conflict of interest shall exclude himself/herself from the decision-making process. Results of the annual review are filed in the employees employment folder.”

“Cedar hill cemetery honors requests for public inspection and distribution of its governing documents, conflict of interest policy and financial statements as required by all federal, state and local laws. Documents are available for public inspection (without charge) at the cedar hill cemetery offices located at 453 fairfield ave., hartford, ct 06114, during regular business hours of 8 am to 4 pm monday through friday. A copy will be provided without charge (other than a reasonable fee for reproduction and actual postage costs) of all or any part of any application or return required to be made available for public inspection to any individual who makes a request for such copy in person or in writing.”
