"champ homes, inc. (chi) continues to be a leader in the cape cod community by housing those who fall through the gaps in other housing programs. Our residents come from a variety of backgrounds with a common characteristic of their life being they were either homeless or near to homeless when they arrived at chi. In 2015 our pilot house 1 and 2 continued to serve the cape cod community with sober home programming and monitored housing. And the homes at our 82 school street campus continued to serve a population from a variety of backgrounds. In 2015, champ homes, inc. Served nearly 100 individuals at the champ homes and 40 more at the pilot houses. One home bayside cottage, in coordination with the cape cod council of churches and the bridge to hope program serves women leaving the barnstable house of corrections who are transitioning back into the community. The ready home serves mostly head injured individuals and most of the case management and life skills work is shared with the community support associates, a for- profit organization serving the head injured under contract with the state head injured program. The alice o'neill house's population houses those in need of short-term and longer term transitional housing; and the lorusso house houses those coming out of chronic homelessness. Champ homes, inc. Provides the housing component of the program and additional case and life skills support. Champ homes continues to increase life skills training with outside contracted consultants to offer educational and job search support to residents who would benefit from these programs. Throughout the year chi also offered daily nutrition, and pathways to health and various programs to residents to help improve their well-being. Chi utilizes "a hand up" rather than a "hand out" approach. Champ homes, inc. Continues to do considerable networking within the communities in order to build a more supportive community response to the root causes of homelessness and to build community wherever and whenever possible. As of december 31st 2015, chi sold its redemption center in yarmouth. Chi residents continue to work at the center and benefit from the foundation of employment opportunities that were created from chi involvement in the start-up of the center and its employment program. The organization is in the process of increasing its life coaching and educational center for its residents. Finally, the organization supports team m25 and its programs. "

(CHI) CONTINUES TO BE A LEADER IN THE CAPE COD COMMUNITY BY HOUSING THOSE WHO FALL THROUGH THE GAPS IN OTHER HOUSING PROGRAMS. OUR RESIDENTS COME FROM A VARIETY OF BACKGROUNDS WITH A COMMON CHARACTERISTIC OF THEIR LIFE BEING THEY WERE EITHER HOMELESS OR NEAR TO HOMELESS WHEN THEY ARRIVED AT CHI. IN 2015 OUR PILOT HOUSE 1 AND 2 CONTINUED TO SERVE THE CAPE COD COMMUNITY WITH SOBER HOME PROGRAMMING AND MONITORED HOUSING. AND THE HOMES AT OUR 82 SCHOOL STREET CAMPUS CONTINUED TO SERVE A POPULATION FROM A VARIETY OF BACKGROUNDS. IN 2015, CHAMP HOMES, INC. SERVED NEARLY 100 INDIVIDUALS AT THE CHAMP HOMES AND 40 MORE AT THE PILOT HOUSES. ONE HOME BAYSIDE COTTAGE, IN COORDINATION WITH THE CAPE COD COUNCIL OF CHURCHES AND THE BRIDGE TO HOPE PROGRAM SERVES WOMEN LEAVING THE BARNSTABLE HOUSE OF CORRECTIONS WHO ARE TRANSITIONING BACK INTO THE COMMUNITY. THE READY HOME SERVES MOSTLY HEAD INJURED INDIVIDUALS AND MOST OF THE CASE MANAGEMENT AND LIFE SKILLS WORK IS SHARED WITH THE COMMUNITY SUPPORT ASSOCIATES, A FOR- PROFIT ORGANIZATION SERVING THE HEAD INJURED UNDER CONTRACT WITH THE STATE HEAD INJURED PROGRAM. THE ALICE O'NEILL HOUSE'S POPULATION HOUSES THOSE IN NEED OF SHORT-TERM AND LONGER TERM TRANSITIONAL HOUSING; AND THE LORUSSO HOUSE HOUSES THOSE COMING OUT OF CHRONIC HOMELESSNESS. CHAMP HOMES, INC. PROVIDES THE HOUSING COMPONENT OF THE PROGRAM AND ADDITIONAL CASE AND LIFE SKILLS SUPPORT. CHAMP HOMES CONTINUES TO INCREASE LIFE SKILLS TRAINING WITH OUTSIDE CONTRACTED CONSULTANTS TO OFFER EDUCATIONAL AND JOB SEARCH SUPPORT TO RESIDENTS WHO WOULD BENEFIT FROM THESE PROGRAMS. THROUGHOUT THE YEAR CHI ALSO OFFERED DAILY NUTRITION, AND PATHWAYS TO HEALTH AND VARIOUS PROGRAMS TO RESIDENTS TO HELP IMPROVE THEIR WELL-BEING. CHI UTILIZES "A HAND UP" RATHER THAN A "HAND OUT" APPROACH. CHAMP HOMES, INC. CONTINUES TO DO CONSIDERABLE NETWORKING WITHIN THE COMMUNITIES IN ORDER TO BUILD A MORE SUPPORTIVE COMMUNITY RESPONSE TO THE ROOT CAUSES OF HOMELESSNESS AND TO BUILD COMMUNITY WHEREVER AND WHENEVER POSSIBLE. AS OF DECEMBER 31ST 2015, CHI SOLD ITS REDEMPTION CENTER IN YARMOUTH. CHI RESIDENTS CONTINUE TO WORK AT THE CENTER AND BENEFIT FROM THE FOUNDATION OF EMPLOYMENT OPPORTUNITIES THAT WERE CREATED FROM CHI INVOLVEMENT IN THE START-UP OF THE CENTER AND ITS EMPLOYMENT PROGRAM. THE ORGANIZATION IS IN THE PROCESS OF INCREASING ITS LIFE COACHING AND EDUCATIONAL CENTER FOR ITS RESIDENTS. FINALLY, THE ORGANIZATION SUPPORTS TEAM M25 AND ITS PROGRAMS. "
