Activity 2

Transitional living the organization's new building has been renovated and houses the 90-bed shelter and twelve (12) units of permanent housing. The grand opening was in january 2006. This new facility allows the lowell transitional living center to offer to its clients a true continuum of care from the outreach component at the street level, through the emergency shelter and all of its supportive services, to the housing units, all under one roof with the benefit of case management and referrals to the appropriate services necessary to address the clients' individual needs.

Activity 3

Case management clients' needs are addressed through a case management team. The team consists of two full-time social workers. Individual service plans (isps) are developed on a one to one basis (client and case manager). These isps are client driven with the case manager acting as a guide making suggestions and referrals as needed. Weekly meetings are held between client and the case manager to assess the progress and address any difficulties along the way. This model has proven to be very successful in helping clients rebuild their lives by setting small attainable goals and then building on that foundation, as they grow more confident.
