Activity 2

Training and employment services - we offer training, placement assistance, and workplace counseling. Our health care candidates receive extensive training and participate in state certification programs including cna, chha, and first aid/cpr. We provide job readiness training. Our staff helps individuals ain the personal and professional skills needed to succeed on the job. Our career choice clients fill entry-level and more skilled positions in sales, data entry, manufacturing, product distribution, and hospitality. Support continues long after the candidate accepts a position. Lifestream cndidates already have a support system in place when they begin a new position. Lifestream helps secure state sponsored childcare and transportation for the participant, eliminating many barriers to successful employment for the typical candidate. The program operates 260 days a year/5 days a week and served 410 clients in the current fiscal year.

Activity 3

Day habilitation - lifestream's day habilitation programs provide medicaid funded support services for adults with disabilities residing throughout southeastern massachusetts. Each center provides an individually tailored, goal oriented program to meet the often challenging needs of people requiring enhanced assistance and support. In addition to experiencd and trained staff members, nursing services and therapy specialists are available at each program location to evaluate and provide clinical supports. Since 1981 the organization has successfully provided individualized medical, therapeutic, and developmental training services to meet the needs of challenged adults. The goals of these services are to maintain and enhance personal health as well as foster the greatest degree of personal independence possible for each person. Each consumer program is individualized for each participant. The program operates 260 days a year/5 days a week and served 186 clients in the current fiscal year.
