The 990 draft copy is shown to the ceo, cfo and the finance committee which includes the president of the board. Once the draft has been reviewed, the final copy is signed by the designated personnel and is mailed to the irs.

Form 990, Part VI, Section B, Line 12C

Every key employee and board member, upon joing the board, is required to sign a conflict of interest statement. Should a conflict arise at any time during the year, they are required to notify springwell's ceo in writing by completion of the provided forms. Also, every year at the annual meeting, the director of hr discusses the conflict of interest policy and the importance of informing springwell about any potential conflicts.

Form 990, Part VI, Section B, Line 15

All positions including that of ceo are assigned a starting salary in accordance with the agency salary plan. The executive committee of the board is responsible for establishing the salary for the ceo. This compensation is determined based on funding available and survey of otehr comparable sized aging service access provider salaries. The process is that hr reviews the salaries for key employees in relation to those of others in the same positions in similar agencies. The most recent review used the data compiled in the mass home care salary survey.

Springwell provides services that help seniors, and individuals with disabilities live at home with dignity and independence. Springwell also supports both family members and professionals who provide care to seniors and individuals with disabilities helping them navigate the emotional, physical and financial maze of caregiving. In addition, springwell works with government agencies, health providers and housing entities to improve their patients and residents' health and satisfaction with care while reducing costs. Case advisors and rns meet with individuals in their home to help them assess their needs, identify supports, access supports and where appropriate to coordinate services. In-home services provided include assistance with personal care such as bathing, dressing, and toileting, assistance with homemaking, grocery shopping and laundry. Assistance is also provided to residents of nursing homes who want to return to living in their own homes in the community. Some of the above services are offered for free, some are subsidized by government programs for frail, low-income individuals and some are provided for a fee. Springwell operates the following government subsidized programs: home care, enhanced community options program, community choices, adult foster care and personal care attendant program. In addition, springwell provides case management of long-term care services for individuals enrolled in senior care and one care programs for individuals enrolled in medicaid. In addition to the above services, springwell is also the designated area agency on aging (aaa) for 8 communities. As the aaa, springwell is the regional hub of services for seniors and is responsible for developing comprehensive, community-based services that meet the needs of all older adults. Springwell is also the designated protective services agency for a 17 town area. In this capacity springwell receives reports of elder abuse, elder self-neglect, and financial exploitation. Staff investigate such reports and endeavor to provide support that helps reduce abuse, neglect and exploitation. Finally, springwell offers nutrition services including home delivered meals, community dining sites, rides to medical appointments, assistance with grocery shopping, friendly visitors and evidence based educational workshops.

Financial Statement Notes

PART X, LINE 2:

Springwell accounts for uncertainty in income taxes in accordance with asc topic, income taxes. This standard clarifies the accounting for uncertainty in tax positions and prescribes a recognition threshold and measurement attribute for the financial statements regarding a tax position taken or expected to be taken in a tax return. Springwell has determined that there are no uncertain tax positions which qualify for either recognition or disclosure in the financial statements at june 30, 2017. Springwell's information returns are subject to examination by the federal and state jurisdictions and generally remain open for the most recent three years.

THE ULTIMATE GOAL IS TO ASSIST OLDER PERSONS IN LEADING INDEPENDENT, MEANINGFUL AND DIGNIFIED LIVES IN THEIR OWN HOMES AND COMMUNITIES FOR AS LONG AS POSSIBLE. SPRINGWELL OFFERS SERVICES SUCH AS RIDES TO MEDICAL APPOINTMENTS, HELP WITH APPLYING FOR PUBLIC BENEFITS, GROCERY SHOPPING ASSISTANCE, FRIENDLY VISITING, TELEPHONE REASSURANCE, AND COMMUNITY EDUCATION OPPORTUNITIES INCLUDING EVIDENCE-BASED HEALTHY AGING PROGRAMS. THE COMMUNITY SERVICES PROGRAM SERVED 5,296 CLIENTS IN FISCAL YEAR 2017. NUTRITION: SPRINGWELL PROVIDES NUTRITIONAL SERVICES TO SENIORS WITHIN THE 8 TOWN SERVICE AREA THROUGH THE HOME- DELIVERED MEAL PROGRAM AND THROUGH SERVING SENIORS AT 10 COMMUNITY DINING SITES WHERE SENIORS CAN RECEIVE A MEAL AND SOCIALIZE WITH NEIGHBORS. SPRINGWELL ALSO PROVIDES NUTRITIONAL RISK ASSESSMENT AND COUNSELING BY A REGISTERED DIETITIAN TO THOSE WHO APPEAR TO BE AT NUTRITIONAL RISK.IN ADDITION TO REGULAR MEALS, SPECIALIZED HOME- DELIVERED MEALS ARE AVAILABLE INCLUDING HEART HEALTHY, KOSHER, CHINESE, AND THERAPEUTIC.
