Activity 2

Children and family services supports 414 families who care for a disabled family member at home. Fhhs helps each client and family set goals via an individual family support plan and provides intensive support for families in crisis. Bi-lingual case managers work directly with latino families on a daily basis, carry out an innovative latino medical program, and coordinate esl classes and a latino parent support group. Fhhs social workers coordinate all of the therapeutic, educational, recreational and support group programs that fall under the umbrella of the "family support center". During the past year there were 124 participants in ongoing therapeutic and recreational programs such as art and music therapy, and 70 participants in educational workshops. In addition, fhhs sponsors a dozen or more community events annually; with 1,473 clients, siblings, parents and family members participating in these popular activities. Recent accomplishments include planning, designing and fundraising for an enhanced family support center designed for program expansion. All family support programs are designed to complement each family member's unique strengths and goals and to promote independence and choice. The new family support center will open in the spring of 2013.

Activity 3

Individual support services provides case management for 30 adults in shared living and 70 men and women living independently. Services include money management, health care management, life skills teaching, communication and social skills, parenting, goal setting and ongoing evaluation. Fhhs staff support each individual to foster personal growth, self-advocacy, independence and full integration into the community. Recent accomplishments include the implementation of a medicaid approved adult foster care program which we expect to grow substantially over the next few years. This program will allow children supported by our family services program to continue receiving services through fhhs as they become adults and transition into adult service models.
