Home first program - the home first program goal is two-fold : 1) to assist homeless persons diagnosed with serious mental illness ( or multi-diagnosed with mental illness, addiction, and/or hiv or other chronic health conditions) in finding , obtaining, and sustaining permanent housing; 2) to address unmet needs of low income persons with such diagnoses who are in permanent housing so they maintain their housing and restabilize. The program is based on a housing first approach using harm reduction strategies and addresses the needs of individuals and families. Services are provided by the mobil integrated services team ( mist) using a scattered site housing model. Comprehensive services include case management,24/7 crisis response, counseling, psychiatric care, medication management , money management ( including services by representative payee) , health care , recovery services and treatment, benefits advocacy , employment readiness and search assistance , community-building and recreational/social activities.through a contract with the developer, a community of friends (acof) that ended in july, 2011 , mist served 58 acof tenant households; over three years a 98% housing retention rate was achieved. The contract was suported primarily by los angeles county homeless prevention initiative funds for 3 years.in 2010-11, 230 individuals were served ( 61 children; 169 adults; all heads of household had disabilities). Geographic areas served are countywide with special focus on hollywood, pasadena, east los angeles, san fernando valley and south los angeles. This program provides a community based solution that enables the organization to assist vulnerable families with challenges of permanent housing despite existing challenges to creating more affordable housing.
