Mission and Program Overview

Mission

Our mission is: "Giving Everyone A New Start."

Since 2010, horizon outreach has connected over 5,000 plus individuals and families - see sched o(2,000 military; 1,345 children indirectly) to resources and supportive services via collaborative partnerships. Of this number, over 808 fathers have increased their workforce development, parenting and relationships skills through the hallmark horizon eagle fatherhood program. The original innovative design was to provide military fathers who had post traumatic stress disorder/other disabilities with strategies to overcome the effects of them on, their relationships, parenting abilities and employability. In the third year of the grant, there was an overwhelming need to serve non-military fathers (at risk young fathers) who were homeless, formerly incarcerated and/or in substance abuse treatment. Therefore, horizon outreach expanded services to serve this population.

The form 990 is completed by a cpa and then forwarded to the director for review. Initial revisions, if any, are made, then the form 990 is sent to all of the board of directors for review. After all revisions are made, the board votes to approve of the final document for filing.

Horizon Outreach has a Code of Conduct policy in place which requires directors, officers, committee members, volunteers and employees to adhere to, and disclose, any financial or other relationships with suppliers, clients or competitors that they may have and the reasons they believe such relationship would not violate the Conflict of Interest provision of the Code. They are required to sign an affirmative statement in which they disclose any potential conflicts of interest and agree to adhere to Horizon Outreach's Code of Conduct.

Form 990, Part VI, Section B, line 15

A consultant was hired to assist Horizon Outreach with establishing policies and procedures for executive compensation as well as conflct of interest policies and other policies. Salary surveys were provided and reviewed by the board to establish reasonable compensation for the Chief Executive Officer based on comparable salaries and benefits with non-profit CEO's of organizations of similiar size and scope as well as for other staff members of the organization. Based on this information, the consultant provided guidance and assistance on establishing an employment contract for the CEO. The Employment Contract, along with the budget for the CEO's compensation, was reviewed and approved by the Board of Directors. Final approval and review of the CEO and employee's salaries were reviewed and approved by the grant funder, The Department of Health and Human Services.

The Organization applies the provisions of FASB ASC Topic 740, Income Taxes, (formerly FASB Interpretation No. 48 (FIN 48), Accounting for Uncertainty in Income Taxes - an Interpretation of FASB Statement No. 109), which prescribes a recognition threshold and measurement attribute for financial statement recognition and measurement of a tax position taken or expected to be taken in a tax return. FIN 48 also provides guidance on de-recognition, classification, interest and penalties, accounting in interim periods, disclosures and transition. The Organization believes that it has appropriate support for any tax positions taken, and as such, does not have any uncertain tax positions that are material to the financial statements.

WE PROVIDED THE FOLLOWING: ONSITE HOUSING/RENTAL & UTILITY ASSISTANCE FOR VETERANS AND THEIR FAMILIES, BLOOD PRESSURE/WELLNESS SCREENINGS, FOOD, ENTERTAINMENT, AND ADDITIONAL AGENCIES WERE PRESENT TO CONNECT FAMILIES TO ONSITE RESOURCES. THIS PROGRAM OFFERS SEMINARS THAT ARE HELD THROUGHOUT THE COMMUNITY AT VARIOUS TIMES IN COLLABORATION WITH OTHER AGENCIES TO ASSIST THOSE IN NEED. SINCE 2011, HORIZON SEMINARS HAVE SERVED OVER 4,329 FAMILIES. THE 4TH ANNUAL FATHER'S DAY EVENT WAS HELD. A TOTAL OF 50 FAMILIES ATTENDED. A TOTAL OF $28,000 ($3,500 EACH) IN SCHOLARSHIPS WERE AWARDED TO 7 FATHER'S AND 1 MOTHER TO ATTEND HOUSTON COMMUNITY COLLEGE'S COMMERCIAL TRUCK DRIVING PROGRAM.

Salary surveys were provided and reviewed by the board to establish reasonable compensation for the Chief Executive Officer based on comparable salaries and benefits with non-profit CEO's of organizations of similiar size and scope as well as for other staff members of the organization. Based on this information, the consultant provided guidance and assistance on establishing an employment contract for the CEO. The Employment Contract, along with the budget for the CEO's compensation, was reviewed and approved by the Board of Directors. Final approval and review of the CEO and employee's salaries were reviewed and approved by the grant funder, The Department of Health and Human Services.
