Our catholic health ministry is dedicated to spiritually-centered, holistic care which sustains and improves the health of individuals and communities. Every day the Foundation partners with the community to obtain resources of time, talent and financial gifts to support the delivery of compassionate quality healthcare services, and every day we experience the generous hearts and willing hands of our donors. Over the past year, $174,610 have been entrusted to the Foundation to help fund improved technology, facility renovations and enhanced patient care services for our community.

Board approves (recorded in minutes), then sent to Foundation Board for approval (also in minutes). A request for Use of Restricted Funds (RURF) form is completed & accompanied by supporting documentation from Materials Management (Purchase Order). Foundation gifts are made via check to the Health System with all supporting documentation attached & given to the health system accounting department. For all non-capital purchases not requiring Board approval, the same process is followed. All RURF forms and supporting documentation are filed in the Foundation Office. The purpose of the grant to St. Joseph Health System, Inc. in the amount of $123,658 was to enable the Health System to purchase needed capital items in order to enhance the delivery of patient care.

This severance plan began 10/16/2010 and ended on 10/15/2012. Part 1, Line 4b: Eligible executives participate in a program that provides for supplemental retirement benefits. The payment of benefits under the program, if any, is entirely dependent upon the facts and circumstances under which the executive terminates employment with the organization. Benefits under the program are unfunded and non-vested. Due to the substantial risk of forfeiture provision, there is no guarantee that these executives will ever receive any benefit under the program.
