Additional Explanations

Form 990, Part III, line 2

Family Time Coaching: This program is run under the Family Visitation program. This offers coaching to children and parents designed to provide skills, training, guidance, and feedback to assist them in reaching their Reunification goals. This is a contracted service with DHHS. Transitional Housing (TH): This program helps youth in the Penobscot, Piscataquis, Aroostook, Washington, and Hancock counties. This program helps young people move into permanent housing. The program offers support services and transitional beds for youth in the above mentioned counties. Rapid Re Housing (RRH): This program help people experiencing homelessness find permanent housing quickly. The program offers Housing identification, Rent and move-in assistance, and case management. This program also helps people avoid returning to homelessness. Helps people increase self-sufficiency. Helps link people link to community resources, is a low-cost intervention and is an effective way to end homelessness. Transitional Living Program (TLP): This is a non-clinical program. This program responds to the need of homeless youth who are without safe family or other natural support systems and require a safe place to live during a time of transition to independent adulthood. The program is designed as a 'next steps' for homeless and at risk youth to move beyond the streets or emergency shelter and into a safe residential environment. One in which they will be able to set goals and develop the living and job skills needed to become independent, productive members of society.

IRS990/Desc0Treatment Foster Care Programs provide children who experience trauma, neglect, and/or behavioral health disorders a safe home with foster families who are licensed by the State to provide treatment level services and support. The children who are served in this program have moderate to high needs and require close supervision and therapeutic interactions and interventions. The staff in this program work with the birth families in order to reunify the family whenever possible. Staff also work with the children and foster parents to assist them in realizing goals in the child's individualized treatment plan. The goals of the Treatment Foster Care Program are to provide a temporary safe and nurturing home and family; to assist the children in dealing with trauma and mental health issues; to work with birth families so that reunification can be realized; and to work to support permanency for all children through reunification, kinship care, adoption, or independent living.

IRS990/ProgSrvcAccomActy2Grp/Desc0Family Visitation: This program provides an office and staff to provide supervised visits between a child and their family of origin. There is also a parenting training component that takes place during the visitation sessions. We provide this service out of 10 offices across 4 DHHS Districts. This is a contracted service with DHHS who holds the contract. The contract is set to provide 5000 units (1hr = 1 Unit) per month split across the four districts based on DHHS youth in custody census.Family Time Coaching: This program is run under the Family Visitation program. This offers coaching to children and parents designed to provide skills, training, guidance, and feedback to assist them in reaching their Reunification goals. This is a contracted service with DHHS.

IRS990/ProgSrvcAccomActy3Grp/Desc0Outpatient mental health and medication management for children, families, and adults in a clinical setting. The goal of this program is to provide psychiatric treatment and/or counseling to individuals to assist them in achieving optimal mental health and functioning. This treatment is provided by Board Certified Child Psychiatrists and Licensed Clinicians.Adult Support Services provide case management and skill development support to adults who are diagnosed with a mental illness; live in the community; and are in need of assistance to maintain their independence or to gain greater independence. Some of our clients are homeless and others have temporary or permanent housing. Staff work with each client to assist them in addressing their most pressing needs whether it be to find an apartment, seek additional resources, maintain healthy relationships, and maintain or improve their physical and/or mental health. Some clients also receive support to learn new skills and to develop a natural support network. Other: An array of services to support children and families including case management for children and families. ECCP: Early Childhood Consultation Program is a clinical service provided by an experienced LCSW. This service provided is evidence based and provided in early childhood day care centers or elementary school settings. Services last approximately 8 weeks in each setting with the focus being on assisting the center or school with individual student of classroom behavioral problems. Much of the first year is dedicated to training the clinicians. In a full year each clinician will serve 12 to 15 centers. This is a 5-year pilot project DHHS is funding.

IRS990/ProgSrvcAccomActyOtherGrp/Desc0Youth Homeless Services our youth homeless shelter the Shaw House, engages youth who are homeless, or at risk of becoming homeless, in achieving stability and a viable plan for the future by providing a safe, stable environment, access to essential services, supportive adult connections, and to advocate for changes that address the needs of homeless youth. In our Youth Homeless Services we offer Street Outreach services is a mobile based program providing access to basic needs including shelter. This program focuses on youth between the ages of 10 to 20 years old, who are disconnected from their family or support systems In our Mobile Diversion and Navigation services provide community-based, short-term support to youth ages 16 to 24 who are currently homeless and in need of support identifying an alternative to the streets and shelter system.In our Mediation services are provided to youth between the ages of 10 to 24 and their natural supports. These services are available virtually through Zoom, or similar programs, and on-site through our day program. The goal of this program is to repair, rebuild and support the relationship between child and family. This allows reintegration into their natural support system.The Youth Shelter program offers emergency services to youth between the ages of 10 to 20 on a 24 hour basis, 7 days a week. We offer youth a place to go when fleeing from domestic violence and are homeless or at risk of being homeless. The shelter provides the youth with food, water, a bed, shower, and access to laundry facilities and basic clothing. The Shelter also offers a Day Program, open from 8am to 7pm, that provides experienced, open and friendly staff to help them stay in school and also overcome their homeless situation.Mason Place Transitional Living Program is an 8 bed, 18 month congregate housing program for homeless young people aged 16.5 to 20. We provide a 24 hour staff and on-site Case Management. The youth who stay here participate in chores, group activities, and skill building opportunities that will increase success in the future independent living.We have Transitional Housing units for youth ages 18 to 24. These units are agency leased. Youth enrolled in this program make income-driven rental contributions and have access to ongoing support services such as assistance with food and technology for educational or vocational needs. Youth also have access to skill building opportunities with one on one and group support to set and achieve their permanency goals.Rapid Re Housing provides flexible, low barrier rental assistance to youth aged 18to 24 (or emancipated minors) who want to live in their own apartment. Youth receive support with the lease-up process and assistance with basic furnishings. As lease-holders, youth make income-driven rental payments directly to their landlord and also have access to on-going support services.Housing Navigation services provided in partnership with Maine State Housing Authority and work to transition youth into their own, leased apartment. This is provided to youth between the ages of 10 to 24, although a youth must be over 18, or an emancipated minor, to enter into their own lease agreements. We offer Case Management for youth that helps them access statewide rental subsidies to assist with paying all or a portion of their rent, as well as financial & consumer education, assistance with the lease-up process.Outpatient Mental Health Clinic. Community Care is a licensed mental health and substance abuse treatment to families, children, and adults of all ages. Our clinic currently serves people who are: recovering from difficult life experiences, including the effects of trauma or abuse, Evidenced based treatment modalities, such as Motivational Interviewing, Trauma Focused Cognitive Behavioral Therapy, Structural Family Therapy to name some. This program is offered to youth and adults of all ages. Different forms of therapy work better for some people than others
