Financial Statement Notes

PART X, LINE 2:

Accounting principles generally accepted in the united states of america require management to evaluate tax positions taken by the foundation and recognize a tax liability if the foundation has taken an uncertain position that more likely than not would not be sustained upon examination by various federal and state taxing authorities. Management has analyzed the tax positions taken by the foundation and has concluded that as of december 31, 2024, there are no uncertain positions taken or expected to be taken that would require recognition of a liability or disclosure in the accompanying consolidated financial statements. The foundation is subject to routine audits by taxing jurisdictions; however, there are currently no audits for any tax periods in progress.

FUNDS RAISED WILL ENHANCE THE FOUNDATION'S ABILITY TO SERVE JPCHC PATIENTS, EMPLOYEES, AND THE COMMUNITY. THE FOUNDATION RUNS A YEAR-LONG "GREATEST NEED ANNUAL FUND" TO RAISE MONEY FOR THE DIVERSE NEEDS OF JPCHC PATIENTS. OVERSEEN BY THE FOUNDATION DIRECTOR AND BOARD, IT WILL REQUIRE ABOUT 30% OF THE FOUNDATION'S TIME AND EXPENSES, MAINLY FUNDED THROUGH DONATIONS FROM INDIVIDUALS, CORPORATIONS, AND PRIVATE FOUNDATIONS. THIS FUND SUPPORTS JPCHC'S CHARITABLE MISSION TO ASSIST PATIENTS AND HEALTHCARE PRACTICES. THE FOUNDATION DIRECTOR AND BOARD OVERSEES FUNDRAISING FOR THE JPCHC FOOD PANTRY, WHICH OPERATES ACROSS 12 INDIANA CLINICS IN CITIES SUCH AS INDIANAPOLIS, KOKOMO, SHELBYVILLE, GREENFIELD, GREENWOOD, ALEXANDRIA, AND ANDERSON. APPROXIMATELY 30% OF THE FOUNDATION'S EFFORTS AND EXPENSES ARE ALLOCATED TO THIS PROGRAM, WHICH IS FUNDED BY DONATIONS FROM INDIVIDUALS, CORPORATIONS, AND PRIVATE FOUNDATIONS. THIS ALIGNS WITH THE FOUNDATION'S GOAL OF SUPPORTING JPCHC'S PATIENTS BY PROVIDING THOSE IN NEED WITH FRESH PRODUCE, CANNED GOODS, AND OTHER ESSENTIAL ITEMS. THE FOUNDATION OVERSEES A "FIELD OF INTEREST FUND" PROVIDING PHILANTHROPIC SUPPORT FOR JPCHC'S SERVICE PROGRAMS, INCLUDING THE DENTAL CLINIC, OPTOMETRY CLINIC, AND JPCHC PHARMACY. THIS WILL ACCOUNT FOR APPROXIMATELY 30% OF THE FOUNDATION'S TIME, MANAGED BY THE DIRECTOR OF FOUNDATION DEVELOPMENT, WHO WILL OVERSEE DONATIONS AND DISBURSEMENTS ALIGNED WITH THE ANNUAL FUNDRAISING GOALS. THE PROGRAM AIMS TO HELP JPCHC PATIENTS BY EXPANDING AVAILABLE SERVICES. THE FOUNDATION OVERSEES AN "EMERGENCY HARDSHIP FUND," WHICH PROVIDES EMPLOYEE-DONATED FUNDS AND PAID TIME OFF (PTO) TO STAFF AND THEIR FAMILIES DURING CRISES. AN INDEPENDENT COMMITTEE, NOT INFLUENCED BY JPCHC LEADERSHIP, WILL SELECT AND AWARD GRANTS. ABOUT 10% OF THE FOUNDATION'S RESOURCES WILL SUPPORT THIS PROGRAM. THE FOUNDATION MANAGES A "PAY IT FORWARD PATIENT GIVING FUND," AN OPTIONAL DONATION PROGRAM FOR JPCHC PATIENTS. FACILITATED BY THE FOUNDATION DIRECTOR AND THE BOARD, IT WILL ACCEPT AND DISBURSE DONATIONS IN ACCORDANCE WITH FOUNDATION POLICY. FUNDED MAINLY BY PATIENT CONTRIBUTIONS, IT WILL REQUIRE LESS THAN 5% OF THE FOUNDATION'S TIME, SUPPORTING PATIENTS' OPPORTUNITY TO CONTRIBUTE TO THE ORGANIZATION'S SUCCESS.
