Heather blankinship, employed by b&c consortia management, llc, is designated as the consortium manager that is in-charge of the management and financial duties and is not compensated separately for her work with the consortium.”

“General membership in the Fluorotelomer Consortium is open to current and prospective recipients of test orders issued for fluorotelomers and related chemistries, namely manufacturers, including importers, and/or processors of the fluorotelomers and related chemistries, all of which have a common interest in promoting compliance with TSCA in a timely and cost-effective manner. Associate membership is available, if requested, to other stakeholders and/or industry trade associations representing fluorotelomer members' interests, contingent upon approval of the Fluorotelomer Consortium Board of Directors and upon the associate member company/organization's (member company) written confirmation to adhere to the Fluorotelomer Consortium Bylaws and any other policies set by the Board of Directors with respect to associate members.”

“The Board of Directors shall consist of not less than three Directors. Each general member company shall appoint its Fluorotelomer Consortium representative to the Board of Directors. Associate member companies are not represented on the Board of Directors. Each member company shall designate in writing to the Executive Director its Board of Directors representative(s), who shall have full authority to represent, vote, and act for such member in all matters coming before the Board of Directors. The terms for Chair and Vice Chair shall be limited to one year, subject to an available extension of these term limits if agreed upon by a majority vote of the Board of Directors, and the Board of Directors shall elect the Chair and Vice Chair at the annual meeting.”

“B&c consortia management, llc performs a detailed review of the form 990 prepared by the certified public accountant firm engaged by the organization. Upon completion of its review, any revision comments are communicated to the cpa firm and an updated copy is provided for final approval by b&c consortia management, llc, upon which time the return is filed with the internal revenue service. A final copy of the filed return is retained by b&c consortia management, llc.”

“B&c consortia management, llc, which has control over management duties of the organization, has a written document retention and destruction policy.”
