“support early identifiation and prevention of kidney disease KEDS (Kidney Early Detection Screening) is a comprehensive program where participants learn their risk factors, and”

“have their weight, blood pressure, blood sugar, urine protein & glumerular filtration rate (GFR - Kidney function measure) checked, and speak with a medical professional”

“about their results. (3) Early education programs to prevent or slow the progression of kidney disease like KIWI (Kidney Interative Workshop & Information) were made widely available as a”

“follow up to screenings. Nationally recognized evidence- based programs promote and develop patient engagement in chronic disease management (CDMSP/DSMP - Chronic Disease/ Diabetes Self Management Programs) and diabetes prevention”

“(DPP - Diabetes prevention program). (4) Kidney patients have continued access to community health navigators for peer guidance and support, empowerment through support groups, nutrition programs and virtual”

“grocery store tours, assistance funds, medical ID jewelry and patient handbooks. (5) Councils formed by NKFH for transplant recipients, donor families, living donors, a coalition on donation and Team”

“Hawaii, help to raise awareness about organ donation and patient health advocacy issues. (6) Youth outreach continues in shift (Shaping Healthy”

“Individuals For Tomorrow) activities held at schools, WOL (Weight of Life), pediatric weight management workshops for families and other community events.”

“(7) Professional workshops and conferences are held each year to provide current kidney information and management support to healthcare providers in Hawaii. (8) Beyond community programs, the NKFH health innovation”

“team has aligned with the broader healthcare landscape to create a more sustainable system level impact. Innovation materializes concepts and strategies to improve health outcomes, control costs, and impact qualify of life. Current”

“activites include: (a) contributing to state intitiatives such as community clinical linkage; (b) transforming physician offices, community health centers and other healthcare providers into quality patient-centered medical”

“homes; and (c) creating a CKD registry for surveillance and longitudinal studies of kidney disease (and eventually other chronic diseases). These innovations reinforce our ability to accomplish our mission throughout Hawaii, especially in”

“remote areas where disparate and disadvantaged populations have limited access to care. (9) Our innovation work has evolved into comprehensive”

“population health management and cultivated the development of clinical services at NKFH. We currently provide care coordination, care management, nutrition services and social”

“support. Our dietitians provide individual counseling, cooking programs, grocery/farm tours, cookbooks and nutrition programs and community support to underscore these services through our volunteer programs.”

“(10) We have increased our focus on tracking outcomes and evaluating impact to ensure quality services and optimize results. Coupled with our registry effort, we hope to capture valuable data that we can analyze and compliment”

“national scientific research endeavors.”

“The change in fund balance represents the decrease in temporarily restricted net assets. Non-cash contributions consist of medical professionals volunteering their time to assist in various programs such as health screenings, kidney”

“education events, research, etc...”

“NKFH recently broke ground on a multi-purpose, multi-story building for its west oahu operations. Plans call for "The Senator Daniel K. Akaka Community Center" to occupy the ground floor, which will be open for community organizations”

“at no or very low cost, complete with a certified kitchen, conference room and library. The rest of the building will be the NKFH Resource Center, named "The Kidney Resource”

“Center", which will house clinical, research and outreach staff. Hawaii residents will be able to participate in screenings, group education and private individual”

“counseling sessions. Medical practitioners will have office space and there will also be a work-out facility, a teaching kitchen and an auditorium. The community center is targeted for completion in 2019, in Kapolei, where there is a”

“concentration of high-risk minorities such as native Hawaiians, Filipinos and Asians. Kapolei's growth makes it the right place for NKFH's growing strides.”
