Review of form 990: the form 990 is prepared by an accounting firm based on the financial statements and information provided by the accounting department of the organization. Prior to filing, the ceo and the board reviewed the form 990.

Form 990, Part VI, Section B, Line 12C

Conflict of interest policy: the clinic established a conflict of interest policy which is presented in the employee handbook. All employees are expected to adhere to this policy. The organization reviews this policy with new hires. Management monitors employees on any issue that may involve a conflict of interest. An annual disclosure form is completed by each board member and monitored by the board chairperson. If a conflict arises, the associated board person must recuse themselves from voting on related matters.
