“(EXPENSES $13,927,035 INCLUDING GRANTS OF $6,601,315)(REVENUE $0) In 2024, FIND's Technology & Development Department continued to advance global health through innovative diagnostics, focusing on expanding access, digital transformation, and the development of next-generation solutions. The department's activities were aligned with FIND's mission to improve health outcomes in low- and middle-income countries (LMICs) by accelerating the availability and adoption of high-quality diagnostic tools. Accelerate access to SARSCOV-2 AgRDTs for Professional Use with manufacturing expansion: FIND supported access to COVID-19 antigen rapid diagnostic tests (AgRDTs) in resource-limited countries by fostering regional manufacturing partnerships. In 2024, the focus was on transferring HIV test technology and building manufacturing capacity with African-based manufacturers, strengthening local production and sustainability. These efforts aimed to ensure a resilient supply chain and reduce dependency on imports. Point Of Care Molecular Diagnostic (POC MDx): FIND invested in four innovative technologies to deliver true and near point-of-care solutions for respiratory panel testing. These platforms were designed to provide rapid, accurate diagnostics at the community level, enabling timely treatment and improved patient outcomes in LMICs. In addition, FIND has conducted an in-depth investigation and assessment of true-POC MDx that led to the launch of feasibility studies on seven small, portable, innovative POC molecular devices. COVID-19 Digital Validation Platform: Building on previous work in computer-aided detection (CAD) for tuberculosis, FIND evaluated digital diagnostics for COVID-19, positioning itself as a leader in digital diagnostic evaluation for the global health community. Technology landscape and DxConnect Test Directory: A core enabler of FIND impact is its ability to generate and disseminate high-quality, open-access IVD technology data that informs decision-making across the diagnostic ecosystem. Our DxConnect Test Directory provides competitive intelligence and detailed information on diagnostic tests and platforms that improve transparency, supports procurement planning, and facilitate innovation. This resource is increasingly used by donors, implementers, and manufacturers to guide strategic investments and product development. FIND has conducted more than 20 technology landscapes covering areas such as antimicrobial resistance, neglected tropical diseases, outbreaks, women's health, and non-communicable diseases. Seq&Treat: FIND accelerated the introduction and adoption of commercial targeted sequencing for the diagnosis of drug-resistant TB (DR-TB). Key 2024 achievements included clinical trials in India, South Africa, and Georgia to support WHO guidance; establishment of a WHO clinical knowledge base (TB-KB) for global sequencing data; proof-of-principle delivery models in India, Indonesia, South Africa, and Brazil; and efforts to address access barriers through pricing collaboration, market mapping, and knowledge sharing. Supporting the development, implementation and evaluation of integrated digital screening tools, and systems for routine collection of syndromic data, at the community level in Rwanda and Kenya: FIND developed and evaluated digital screening approaches at the community level. By integrating disease screening into digital platforms and enhancing contact tracing modules for tuberculosis, FIND demonstrated the feasibility of decentralized community-level screening in Rwanda and Kenya. Shaping the Global Agenda for Genomics: FIND contributed to shaping the global genomics agenda by mapping pathogen genomic sequencing investments and identifying gaps and opportunities for strategic collaboration. “(EXPENSES $5,232,718 INCLUDING GRANTS OF $422,632)(REVENUE $0) In 2024, the FIND Access Department advanced its mission to ensure equitable access to quality diagnostics through strategic initiatives across multiple countries. Our work was organized around four key pillars: Laboratory System Strengthening, Diagnostic Network Optimization (DNO), Non-Communicable Diseases (NCDs), and Training & Capacity Building. This report summarizes the progress and achievements within these thematic areas throughout the year. Laboratory System Strengthening FIND enhanced the quality and sustainability of laboratory services through direct technical assistance. In Bihar, India, the Laboratory System Strengthening project provided support across all 38 districts, mentoring Medical Colleges and implementing quality management systems. This included training over 1,300 staff and conducting 350 site visits, which significantly increased the availability of diagnostic tests. In Indonesia, FIND assisted three referral laboratories in preparing for ISO 15189 accreditation and also led the coordination of the National EDL (NEDL) development in collaboration with the Ministry of Health and other stakeholders. This involved extensive stakeholder engagement and technical working group sessions with relevant Ministry directorates and subject matter experts, contributing to the broader ASEAN Diagnostic Security and Self-Reliance framework. In Bangladesh, technical assistance strengthened TB lab services through SLMTA mentoring at six reference labs, the introduction of a new Laboratory Information Management System, and building national capacity for quality assurance. Diagnostic Network Optimization (DNO) Using geospatial analysis and evidence-based modeling, FIND supported governments in rationalizing diagnostic networks for improved efficiency and access. In Indonesia, the DNO initiative focused on optimizing the TB diagnostic network by identifying optimal platform placement and streamlining sample pathways. In Bangladesh, the project mapped facilities and modeled optimized configurations for TB, HIV, and Malaria services to inform the upcoming National Strategic Plan. In Lao PDR, technical assistance was provided began to develop a national operational plan for an integrated sample transportation system. Globally FIND developed and piloted DxPulse, a new open-source diagnostic network visualization and analysis tool integrated within DHIS2. The pilot in Kenya in 2024, involving two counties, demonstrated its potential for improving data-driven planning and monitoring of diagnostic networks. Non-Communicable Diseases (NCDs) FIND expanded its focus on NCDs by generating critical evidence for policy and investment. The ACCEDE project in South Africa and Kenya, an operational research initiative, is building a local investment case for equitable adoption of continuous glucose monitoring (CGM) in public health systems. The project, co-created with Ministries of Health, combines clinical research and health economic modeling. It also includes a strong capacity-building component, having trained over 670 healthcare professionals across both countries through online and in-person sessions. Training and Capacity Building Building human resource capacity was a key cross-cutting theme. FIND expanded its online training portfolio through the FIND Training Academy. Courses launched in 2024 included "Introduction to Diagnostic Network Optimization (DNO) 2.0," "Introduction to Next-Generation Sequencing and Bioinformatics (INSEQBIN), "Online Training for AMR Scorecard Assessors." “(EXPENSES $4,890,020 INCLUDING GRANTS OF $627,673) (REVENUE $0) TB Programme Activities - 2024 Summary 1. TB Fluorescent LAM Test Development This project focused on advancing fluorescent-based LAM assays to improve diagnostic sensitivity for TB in HIV-positive populations. In 2024, prototype assays were refined and evaluated in controlled laboratory settings. Preliminary data demonstrated enhanced detection rates compared to conventional LAM tests, paving the way for field validation. 2. TPP for Centralised TB Assays The initiative aimed to develop Target Product Profiles (TPPs) for centralised TB diagnostic platforms to guide manufacturers and procurement agencies. During 2024, consensus workshops were held with global stakeholders to finalise specifications for accuracy, throughput, and cost-effectiveness. The resulting TPPs were published and disseminated to inform future product development. 3. TB-CAPT (Close the gap, increase Access, Provide adequate Therapy) TB-CAPT sought to expand access to TB diagnosis and treatment in high-burden countries through integrated service delivery. In 2024, the programme scaled up TB testing and primary health centres and improved linkage-to-care pathways in multiple regions. These efforts resulted in thousands of additional patients initiating timely therapy. Three separate trials were conducted to increase access and diagnosis in symptomatic adults, PLHIV and people with drug-resistant TB. These studies have lead to policy changes and adoption in countries. 4. Predicting the Future: Incipient Tuberculosis (PreFIT) PreFIT aimed to identify biomarkers and predictive models for incipient TB to enable early intervention. Over the year, large cohort studies in South Africa, Mozambique and Uganda were initiated and data analysis frameworks established. Multiple novel tests were evaluated for performance in diagnosing active TB and predicting progression to TB. The primary test evaluated was Cepheid's Xpert Host Response transcriptomic test. 5. Bioneer Accupower RFIA Clinical Evaluation This project evaluated the laboratory analytical and clinical performance of the Bioneer Accupower Rapid Fluorescent Immunoassay for TB detection. In 2024, multi-site trials were completed across diverse epidemiological settings in Peru, Kenya and Ukraine. Specimens have been banked, and a new version of the test will be released in 2025 to complement results. This data will be used to support regulatory submission plans in 2026. 6. FEND for TB (Feasibility of Novel Diagnostics for TB in Endemic Countries) FEND for TB assessed the operational feasibility of introducing novel TB diagnostics in resource-limited settings. During 2024, pilot deployments were conducted in endemic countries, focusing on performance and accuracy of prototype early-stage diagnostics. Over 300 products were assessed through scouting, and over 60 products entered into evaluations in the lab and/or clinic across 5 countries. 7. Rapid Research in Diagnostics Development for TB Network (R2D2 TB Network) The R2D2 TB Network aimed to accelerate collaborative research on innovative TB diagnostics. In 2024, the network expanded to include new academic and industry partners and launched joint studies on next-generation assays. These collaborations produced early prototypes and shared data platforms to speed development.
