Form 990 is reviewed and approved by the finance committee prior to submission. A copy is then provided to each governing board member.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

Each trustee, principal officer and member of a committee with board-delegated powers shall annually sign a disclosure statement which affirms that such person: a) has received a copy of the conflict of interest policy, b) has read and understood the policy, c) has agreed to comply with the policy, and d) understands that the organization is a charitable organization and that in order to maintain its federal tax exemption it must engage primarily in activities that accomplish one or more of its tax-exempt purposes. In addition, each signatory shall notify the organization should any potential conflict arise subsequent to the submission of the disclosure statement for a given year, and shall file a revised disclosure statement detailing such conflict.
