Our mission is to achieve health care equity and promote well-being for the lgbt and other underserved communities and people living with and affected by hiv.

Improving the lives of people affected by hiv disease, reducing the incidence of hiv infection; and advocating for fair and effective hiv-related public policy.

THE PROGRAM SAVES MONEY BY HELPING CLIENTS TO STAY IN THEIR OWN HOMES THEREBY REDUCING THE NEED FOR COSTLY SKILLED NURSING OR EXTENDED CARE PLACEMENTS, AND BY REDUCING THE BARRIERS TO EFFECTIVE HIV TREATMENT THEREBY HELPING LIMIT HIV-DISEASE PROGRESSION. ADDITIONAL MAJOR OUTCOMES OF THE PROGRAM INCLUDE ADDRESSING ADDICTION AND MENTAL ILLNESS WHICH UNDERMINE STABILITY, HELPING CLIENTS MANAGE A VAST ARRAY OF CO-OCCURRING DISEASES AND DISORDERS, DECREASING THE RISK OF TRANSMITTING HIV AND OTHER SEXUALLY TRANSMITTED INFECTIONS.
