Casa Colina is the sole member of the organization.

Form 990, Part VI, Line 7A Members or stockholders electing members of governing body

The sole member of the filing organization is Casa Colina, Inc. The sole member shall elect the board of directors unless otherwise authorized by the bylaws. Currently, the bylaws authorize certain identified members of the board of directors within the filing organization and related organizations. Actions may be taken by the statutory member on behalf of the filing organization without a meeting if authorized by the board of directors of the statutory member. Authorization is required to be memorialized in writing and signed by an authorized officer of the member and filed with the secretary of the filing organization. The member may increase or decrease the number of board members. The member has the final approval of any changes to the bylaws.

Form 990, Part VI, Line 11B Review of form 990 by governing body

The Form 990 was reviewed by an independent accounting firm for accuracy and completeness. A paper copy of the Form 990 is made available for review by the Board of Directors prior to filing with the Internal Revenue Service.

Form 990, Part VI, Line 12C Conflict of interest policy

The Governance committee of the Board reviews disclosure forms on an annual basis. In addition, the ratio of interested vs. disinterested directors is closely monitored. Interested directors are not allowed to hold office or serve on the audit committee. Finally a protocol has been established that prevents the conversion of a current disinterested director to an interested director.

The filing organization relied on a related organization's methods to establish the top management official's compensation. The related organization, Casa Colina, Inc., used the following methods to establish compensation for the CEO: The Board maintains a compensation committee that conducted an independent compensation consultation and compensation study. Once approved by the Board of Directors, the CEO is then offered a written employment contract.

IRS990/Desc0Padua Village is a long-term residential program comprising two homes that serve adults with developmental disabilities. Designed for individuals who have completed rehabilitation yet require continued supervision, the program offers a least restrictive environment where residents can participate in activities that promote self-reliance and personal growth. During the year, Padua Village averaged 11.0 residents per month. Apple Valley and Rancho Pino Verde (Lucerne Valley) are dedicated to supporting individuals with traumatic or acquired brain injuries (TBI or ABI). These safe, compassionate environments are designed to help residents regain independence, build essential life skills, and foster community connections while honoring their dignity and self-esteem. Residents receive 24-hour supervision from a board-certified medical director and a multidisciplinary staff of administrators, nurses, neuropsychologists, case managers, dietitians, activity coordinators, and residential aides. All staff are trained in crisis prevention intervention and behavioral management. Residents also have access to Casa Colina's complete continuum of medical and rehabilitative care. During the year, the Apple Valley program averaged 22.7 residents per month, while Rancho Pino Verde averaged 34.8 residents per month.
