Casa Colina, Inc. is the sole member of the organization.

Form 990, Part VI, Line 7A Members or stockholders electing members of governing body

The sole member of the filing organization is Casa Colina, Inc. The sole member shall elect the board of directors unless otherwise authorized by the bylaws. Currently, the bylaws authorize certain identified members of the board of directors within the filing organization and related organizations. Actions may be taken by the statutory member on behalf of the filing organization without a meeting authorized by the board of directors of the statutory member. Authorization is required to be memorialized in writing and signed by an authorized officer of the member and filed with the secretary of the filing organization. The member may increase or decrease the number of board members. The member has the final approval of any changes to the bylaws.

The Form 990 was reviewed by an independent accounting firm for accuracy and completeness. A paper copy of the Form 990 is made available for review by the Board of Directors prior to filing with the Internal Revenue Service.

Form 990, Part VI, Line 12C Conflict of interest policy

The Governance Committee of the Board reviews disclosure forms on an annual basis. In addition the ratio of interested vs disinterested directors is closely monitored. Interested directors are not allowed to hold office or serve on the audit committee. Finally, a protocol has been established that prevents the conversion of a current disinterested director to an interested director.

IRS990/Desc0The Transitional Living Center provides a pleasing structured environment both in a 24 hour residential care and day treatment program where those with brain injuries, spinal cord injuries, and neurological trauma can safely work on developing their highest attainable level for independent and productive living. Residential patients are under the careful supervision of a comprehensive, interdisciplinary team comprised of physicians, neurophysiologists, behavioral specialists, speech-language pathologists, physical and occupational therapists, nurses, and clinical case managers. In this intensive therapeutic environment, the therapy team uses a variety of rehabilitative strategies to maximize each patient's full potential. Focus is placed on physical restoration of functions, cognitive skill building, social/behavioral shaping, community and psychological social adjustment with the goal of reintegration into the home and community. Family members are encouraged to participate, if appropriate, alongside our professionals to fully witness and understand the necessary techniques to facilitate a smooth transition and to prepare them for their role in the individual's future care needs. While therapies take place at the Transitional Living Center campus, patients also interact in "real life" surroundings. Community sites are incorporated to practice skills in functional situations and to facilitate a transition to the site of discharge.
