Any questions or concerns will be noted and addressed, and management staff will ensure that changes are incorporated into the form 990 as appropriate

A questionnaire is provided each year to all board members and staff to determine if there is a conflict of interest. The questionnaire asks board and staff members to disclose existing conflicts and reminds them of the responsibility to disclose any future potential conflicts of interst. Annual board meetings review the policies and discuss hypothetical situations that could result in a conflict of interest, and then discuss how the board would manage that potential conflict so that any future situations could be handled with more ease.
