Mission

Project open hand's mission is to improve health outcomes and quality of life by providing nutritious meals to the sick and vulnerable, caring for and educating our community. Meals are medically tailored to address the needs of older adults, people living with chronic disease and vulnerable communities experiencing food and nutrition insecurity. Our medically tailored meal intervention includes meals that are catered to addresss specific dietary needs paired with education provided one on one or in a classroom setting by registered dietitians.

Professional Tax Preparers FORWARD THE FORM 990 TO THE Controller and CEO. AFTER REVIEWING THE return, THE Controller FORWARDS it TO THE AUDIT COMMITTEE and the board of directors FOR THEIR REVIEW PRIOR TO FILING. ALSO, BOARD MEMBERS ARE ENCOURAGED TO REVIEW THE FORM 990 AND TO FORWARD THEIR QUESTIONS TO THE CEO and controller. THE Controller OR professional tax preparers ADDRESS THE QUESTIONS FROM THE AUDIT COMMITTEE AND THE BOARD.

Form 990, Part VI, Section B, line 12C

POH's conflict of interest policy is written in the bylaws and the employee handbook. The board and the human resources department regularly monitor and enforce the conflict of interest policy when potential conflicts of interest situations arise. The policies handle conflicts of interest both directly and indirectly in the board governance documents and in the employee handbook. Handbook policies cover all staff, but especially those in leadership and those handling any financial transactions. Under the Whistleblower policy, concerns can be brought forward by any person to the human resources director or manager, Controller, or CEO and chair, co-chair, or secretary of the board. Externally, to the auditors or any of the regulating authorities. Conflicts are reviewed in connection with Human Resources, Finance, and the Executive Director, in general the conflicts would be resolved at this level. If not, the board or external partners will be brought in, if necessary. The organization does not allow for family members to supervise one another. In other cases, persons with conflicts of interest would be removed from the decision making process/ function found in conflict or if that is not possible, objective parties would be added to the decision making process.

Compensation for the executive director is determined by the board based on the compensation committee recommendations. Compensation for the chief financial officer is determined by the executive director and approved by the board. Compensation for other directors and key employees is determined and approved by the executive director.

IRS990/Desc0Older adults (60+ years) and adults with disabilities (18-59 years) enjoy a warm, nutritious meal at congregate sites operated by Project Open Hand's Community Nutrition Program and situated in community centers located across San Francsico. As the largest congregate meal provider for these populations in San Francisco, these meals are available 365 days a year through our Community Nutrition Program. Older adults and adults with disabilities who visit our sites experience decreased risks of isolation and loneliness and enjoy the positive effects of social integration and improved quality of life. Since the COVID-19 pandemic, our congregate meal services have a takeaway model in addition to the dine-in option. This allows for client choice and better meets the needs of our community.

Our medically tailored services are accessed through our grocery centers in San Francisco and Oakland, satellite grocery center locations in San Francisco, and weekly home delivery of meals and groceries. As part of our nutrition interventions, all clients have access to not only our meals and groceries but educational services offered through our Nutrition Services team. This includes individual counseling sessions with our Registered Dietitians, cooking demonstrations, food demonstrations and various other classes and resources. Taking a client-centered approach in our nutrition services, our team is also available to provide added support to clients needing assistance and linkage to other beneficial resources that address various social determinants of health.

POH and CalFIMC lead policy and advocacy efforts to integrate medically tailored meal (MTM) interventions into health care services, eligible for payments. This advocacy has been instrumental in the CalAIM expansion to include medically tailored nutrition interventions into Community Supports in California for eligible Medi-Cal populations. Subsequently, Project Open Hand has been working with managed care plans in Alameda County, Contra Costa County and San Francisco County since 2022 broadening its reach.
