OTHER PROGRAM SERVICES 4: AGING & DISABILITY RESOURCE CONNECTION: We assisted with individuals with enhanced information & referral services, options planning, short term service coordination and person-centered transition/diversion support. OTHER PROGRAM SERVICES 5: DDARC POWERED & PREPARED:Provided a range of services & supports to individuals who are at risk during public safety power shut offs and other wind events where the power may be shut off. services included the creation of an emergency preparedness instruction and workbook, guidance on how to prepare an emergency supplies kit, education and application assistance for the medical baseline program and outreach to community partners. OTHER PROGRAM SERVICES 6: Youth & Family Support. Provided information, training & support around disability issues to families of children with disabilities. Activities included conducting consultations, facilitating support groups, provided trainings and workshops and attended critical meetings with families when needed to reach the overall goal of providing guidance and assistance to families seeking when they are seeking appropriate services and resources for their child or children.
