Activity 2

Behavioral health (mental health services and addictions treatment): the effort has built a network of primary care clinics that provide a "synergy" of services, with our doctors, treating the whole person with a treatment model called integrated behavioral health the effort operates one of the largest substance abuse treatment programs in sacramento through out-patient services, a medically-supported detoxification facility, and residential treatment. Through a merger with family service agency in 2005, the agency has also become a leader in mental health services and treatment of co-occurring disorders. Crisis line: the effort operates a 24 hour crisis line for people contemplating suicide, and also provides advice to individuals who come in contact with people considering suicide. The effort also provides a parent support line to assist parents with their parenting skills - especially during crisis or stressful situations. Home visitation: the effort provides home visitation services to over 160 families each year and assists with case management, linkage to services, eligibility applications, access to medical care and parenting skills. The birth & beyond program has reduced child abuse referrals of participants by 38%.

Activity 3

Services for homeless: the effort operates many successful programs, including the frequent user program to address the needs of patients who frequently use the emergency department. The effort medical clinic serves as a medical home and patients receive on-going medical services, addictions treatment, and mental health services at the effort clinic. Emergency room utilization by program participants has been reduced by 52%. The effort has a strong history of partnership in its very successful interim care program. The interim care program is a program for homeless patients when they are discharged from the hospitals and are well enough to go home, but need on-going rest and follow-up treatment. The effort provides case management, nursing oversight, and medical care.
