Aurora Health Foundation Inc. (AHF) is organized as a non-stock, nonprofit corporation with a sole corporate member of Aurora Health Care Central, Inc.

Certain decisions of the governing body of AHF are subject to approval of Aurora Health Care, Inc.

The 2016 Form 990 was reviewed by senior management of AHC. A copy of the final return was made available to the AHC Board of Directors. The governing body of AHF has approved review of the Form 990 by the AHC Board of Directors, as AHC has financial oversight over the operations of AHF.

Officers, directors and key employees are required to complete a Conflict of Interest Statement on an annual basis. Aurora Health Care leadership reviews these statements for compliance with the established policy. When a conflict is discovered, the governing body determines if such a conflict warrants a restriction on the capacity of the board member involved.
