THESE TEAMS PROVIDES BARRIER IDENTIFICATION AND REDUCTION, PERSON-FIRST HOUSING PLANS, AND WRAPAROUND SERVICES INCLUDING CRISIS MANAGEMENT, EVICTION PREVENTION, LIFE SKILLS TRAINING, TENANCY REQUIREMENTS TRAINING, BENEFITS ESTABLISHMENT, AND OTHER CRITICAL COMPONENTS FOR PARTICIPANTS TO BECOME AND REMAIN HOUSED.
