Form 990, Part VI, Line 11B: Form 990 Review Process

The organizations independent cpa provides the form 990 to the agency after completing preparation. A copy of the form 990 is distributed to all of the board members who have a chance to review, comment, and propose changes before filing.

Form 990, Part VI, Line 12C: Explanation of Monitoring and Enforcement of Conflicts

All employees and board members are provided with the organizations conflict of interest policy. They are required to disclose any actual or potential conflicts of interest. At least annually the organization reviews compliance with the policy.

Form 990, Part VI, Line 15A: Compensation Review & Approval Process - CEO, Top Management

The executive directors salary is determined by the executive committee of the board of directors, whereas the salary for the finance director is determined by the executive director based upon market compensation as described below. If it is determined that the organization needs to hire outside of the salary ranges established, approval is required from the board of directors. These processes are done in collaboration with the human resources director. Salary ranges are in place for all of these positions and are updated annually. Individual positions are market priced at least every two years using the following surveys: the mbl group, oregon labor market information system, and americas career infonet. Actions to determine compensation are documented through the use of an excel spreadsheet.

Form 990, Part III, Line 4D: Other Program Services Description

Other program services 4: housing development program: development - the acquisition and new construction of affordable housing for low-income individuals, families, veterans, seniors and people with disabilities. Other program services 5: property/asset management - access has an ownership interest in 165 units of affordable rental housing, located in jackson county. Of this, 52 units are managed by a third-party management company while the remaining 113 units are self-managed. In addition to the self-managed units, access manages another 24 units of housing owned by ashland community land trust and the southern oregon lion's sight & hearing center. Other program services 6: housing counseling - provides pre and post purchase counseling and education to participants enrolled in the following programs; the oregon foreclosure avoidance program, national foreclosure mitigation counseling program, realizing the american dream, e-home america, and a variety of down-payment assistance programs. Over 491 individuals were helped with foreclosure issues or to purchase their first home. 93% successfully maintained thier homeownership without foreclosure. Other program services 7: community services block grant programs: the grants provide funds to operate several programs. The medical equipment loan program provides medical equipment on loan to seniors and disabled persons at no charge as long as there is a need. Hospital beds, walkers, crutches, and wheelchairs are examples of equipment available. The information and referral program provides information via the telephone to persons seeking assistance regarding the organizations programs and other human services agencies in jackson county. Community planning provides for assessment of community wide needs in order to provide more effective services and programs. It also coordinates jackson countys continuum of care for homeless residents. Program support provides additional funding to support administrative and program costs of other programs. Other program services 8: other programs: the olsrud family nutrition center provides operation of a community nutrition center used to provide a meeting facility with catering options to other agencies in the community and a commercial kitchen used to prepare meals distributed by food and friends (a meals on wheels program).

Part X : FIN48 Footnote

The organization files income tax returns in the u.s. Federal jurisdiction. The organization is no longer subject to u.s. Federal income tax examinations by tax authorities for years before june 30, 2011. Currently, there is no examination or pending examination with the internal revenue service (irs).the organization adopted the provisions of fasb asc 740-10 accounting for uncertainty in income taxes, on july 1, 2009. As of june 30, 2015, there are no tax positions for which the deductibility is certain but for which there is uncertainty regarding the timing of such deductibility.

THE COMMODITY SUPPLEMENTAL FOOD PROGRAM PROVIDED COMMODITY FOOD BOXES TO 325 LOW-INCOME SENIORS EACH MONTH. SEVEN ACCESS FOOD SHARE GARDENS ACROSS THE COUNTY PRODUCED OVER 50,000 POUNDS OF ORGANIC PRODUCE THAT WAS DISTRIBUTED THROUGH THE ACCESS NETWORK OF PANTRIES. OVER 40 VOLUNTEER TRAINED COOKING SKILLS EDUCATORS PROVIDED COOKING SKILLS AND TASTINGS THROUGH THE COOKING SKILLS EDUCATION PROGRAM TO OVER 5,000 INDIVIDUALS AT PANTRIES, STORES, FARMERS MARKETS, EVENTS, ETC. THE FRESH ALLIANCE PROGRAM RECOVERS MILK, PRODUCE, DAIRY PRODUCTS, AND FROZEN MEAT FROM LOCAL GROCERY STORES ON OR BEFORE THE PULL DATE. DURING THE FISCAL YEAR, OVER 25,000 PEOPLE WERE PROVIDED ASSISTANCE.
