Conflict of interest policy compliance Part VI line 12C

Before entering into a transaction that may contain a conflict of interest, the david carr memorial fund board of directors requires each board member or officer to disclose that transaction. The boards personnel committee considers each disclosure and takes the appropriate action to bring the organization into compliance with the code of ethics. In addition, each board member and officer is required to annually complete a conflict of interest questionnaire and return it to the president of the board.
