“The hplf's form 990 is prepared by the treasurer, an officer or director, or an external accountant, in consultation with the board of directors and relevant officers. A draft of the completed form 990 is circulated to all voting members of the board prior to filing. Board members are given the opportunity to review the document and its corresponding attachements, ask questions, and provide feedback. Any necessary revisions are made based on board input. The final version is then approved for filing by the treasurer or designated officer.”

In the case of the treasurer, amy mills, who aslo serves as a trustee of the holbrook public library, she was asked to review and sign the conflict of interest policy to acknowledge her responsibilities and potential areas of conflict. Although the policy did not require annual disclosures of financial or organizational interests, the hplf monitored compliance on a case-by-case basis. Potential conflicts were identified through regular communication with officers and board members and addressed through recusal from discussions or decisions when appropriate. The board retains discretion to revisit and revise the policy to strengthen disclosure and enforcement policies.”
