Form 990-ez, Part III, Line 28

The healthy babies home visiting program is for nuring home visitations for at-risk new born children to assess learning skills, and other potential medical problems. Home visiting staff were able to connect with high risk mother/infant pairs. The visitations were for hospital vistis and for follow up visits at the child's home.

Form 990-ez, Part III, Line 29

The child in crisis program provides funds for medical related expenses for children who exceed the coverage of the family's medical insurance. This program assists families with funds to address the need for prescriptions, prosthethics, equipment, rehabilitation, emergenacy transportation, travel to distant medical facilities, meals, and motel expenses while the child is receiving care in a tertiary care setting away from home. During this year, the program was able to assist 19 families whose children had encountered a severe illness or injury.

Form 990-ez, Part III, Line 30

United way grant was received to provide for a children's community care consultant. The consultant's duties were to collaborate with united way agencies that assist families in need, to consult with the united way board regarding thier grant process, offer perspective of the vialability of a applicant's strategies and outcomes, consult with united way president/ceo regarding community agency needs, and collaborate with care providers where agencies might need assistance.

Other program services include love & logic classes on parenting skills; a grant to create integrated medical homes to provide prevention, early identification, and intervention care for mental illness targeted for children covered by medicaid who demonstrate a certain degree of emotional health disturbance; and a school nurse program for students at-risk and in need of medical treatment.
