It also includes requiring all persons subject to the conflict of interest policy annually to sign a statement affirming that they are familiar with the terms of the conflict of interest policy. The policy requires all persons subject to the policy to provide annually written responses to a questionnaire entitled "conflict of interest disclosure statement." all persons subject to the conflict of interest policy are obligated by the policy promptly to inform the chair of the board of directors of any material change that develops with regard to their disclosure statement, which is distributed to directors and officers at the annual meeting of the board of directors.”
