IRS990/ProgSrvcAccomActy2Grp/Desc0FEE BASED SERVICES:ONE OF OUR LARGEST FEE FOR SERVICE PROGRAMS IS THE ROADS TO COMMUNITY LIVING (RCL). CFI CONTRACTS WITH PIERCE AND THURSTON COUNTIES TO PROVIDE SERVICES TO INDIVIDUALS WHO HAVE BEEN LIVING IN SOME TYPE OF INSTITUTION (NURSING HOME, HOSPITAL, ETC) TO ASSIST THEM IN TRANSITIONING BACK OUT INTO THE COMMUNITY. WHETHER THEY'RE RETURNING TO AN ESTABLISHED HOME, A NEW APARTMENT, OR ADULT FAMILY HOME, THIS BODY OF WORK IS VERY DETAILED AND TIME SENSITIVE REQUIRING ATTENTION TO DETAIL, LISTENING TO THE NEEDS OF THE PARTICIPANT AND OFTEN ADVOCATING TO MAKE SURE THE COUNTY IS ALSO HONORING THE INTENTION OF THE PROGRAM AND INDIVIDUAL NEEDS. WE ALSO CONTRACT WITH DVR TO PROVIDE ASSESSMENTS, IL SKILLS TRAINING AND COMMUNITY ACCESS ON A CASE BY CASE BASIS. CFI HAS BEEN A LEADER IN OUR COMMUNITY BY PROVIDING ONE-ON-ONE AND GROUP TRAININGS IN REGARDS TO EMERGENCY PREPAREDNESS. WE HAVE RECEIVED SOME FUNDING FROM THE COUNTY EMERGENCY MANAGEMENT AND UASI AS WELL AS SOME STATE FUNDING FOR A STATEWIDE PROJECT.

IRS990/ProgSrvcAccomActy3Grp/Desc0YOUTH TRANSITION PROGRAM: THIS PROGRAM ASSISTS IN PREPARING YOUTH FOR THE TRANSITION FROM HIGH SCHOOL TO ADULT LIFE. EACH SUMMER, CFI RUNS A TRANSITION YOUTH SUMMER WORKSHOP WHICH FOCUSES ON CONTINUED EDUCATION, VOCATIONAL GOALS, AND PROGRAMS IN THE COMMUNITY WHICH PROVIDE VARIOUS SERVICES ONCE A YOUTH IS OUT OF HIGH SCHOOL. FROM THIS PROGRAM, MANY YOUTH SIGN UP FOR INDIVIDUAL SERVICE AND/OR PEER GROUP INCLUSION. DURING THE SUMMER OF 2014, CFI SECURED ADDITIONAL MONEY THROUGH DVR TO HOST A STATEWIDE TRANSITION YOUTH SUMMER CONFERENCE. ALL CILS IN WA STATE WERE INVITED TO BRING YOUTH AND THE YOUTH LEADER TO SEATTLE FOR THIS WEEK-LONG EVENT. CFI PAID FOR TRANSPORTATION (AIR, TRAIN, BUS OR MILEAGE). THE EVENT WAS HELD AT THE SEATTLE UNIVERSITY CAMPUS WHERE THE YOUTH WERE HOUSED IN DORMS, HAD FOOD CARDS FOR THEIR MEALS IN THE UNIVERSITY CAFETERIA, AND A SCHEDULE TO MEET IN AN ASSIGNED CLASSROOM. CFI STAFF AND SOME STAFF FROM TWO OTHER CILS PROVIDED MOST OF THE INSTRUCTION AND COMMUNITY PARTNERS FROM THE UNIVERSITY; WORK SOURCE, DVR, WATAP, ETC WERE SCHEDULED THROUGHOUT THE WEEK TO SHARE ABOUT THE SERVICES AVAILABLE IN THE COMMUNITY. ON THE LAST DAY, AN INSPIRATIONAL SPEAKER AND COMEDIAN WHO EXPERIENCES A DISABILITY HERSELF, CAPPED OFF THE WEEK. THREE YOUTH WHO HAD BEEN THROUGH OUR WORKSHOP PREVIOUSLY AND HAD DEMONSTRATED LEADERSHIP ABILITIES ALSO PROVIDED PEER MENTORSHIP TO THE YOUTH. EACH STUDENT LEFT WITH NEW SENSE OF PURPOSE, A CERTIFICATE OF COMPLETION AND A GIFT CARD EARNED THROUGH PARTICIPATION IN THE PROGRAM.
