“Application for membership sec. 1 each applicant shall sign the regular printed form, carrying the pledge of service, and shall have it signed by two (2) members in good standing of the department and have one (1) year or more of service. Applications shall be presented to any officer and he/she shall list the date and time the application was received. It shall be the duty of the officer to present the application(s) in the order they were received and in such a manner that only one application per opening be presented for approval at a time. Sec. 2 each applicant for membership shall, if there be a vacancy in the membership, will meet with the line officers. After meeting with the line officers the applicant will be placed on six (6) month probation effective immediately. After six (6) months, the applicant shall be voted upon for active membership. A two-thirds (2/3) majority vote of members present is necessary for election; otherwise, the probationary member shall be dropped from the roll of the department. A probationary member may be dismissed at any time during the probationary period upon a 2/3 vote of members present at any regular business meeting or at a special called meeting. Sec. 3no rejected applicant, new or probationary, shall be allowed to file a new application for a one (1)year period. Sec. 4every of applicant is required to undergo a full physical and drug screen (hereafter known as "pre-election exam") prior to a vote on his application to become a probationary member of this department. The scope of this pre-election exam shall be determined by the department, and the department shall present applicant with the proper forms, name and location of the facility and/or medical doctor that will be performing such tests and the date and time he/she shall be present to undergo such tests. Should applicant not pass the drug screen portion of the exam and question the result, he/she shall be entitled to a second screen. A second failure shall constitute failure of the pre-election exam. Once this pre-election exam has been performed, the facility and/or medical doctor is only required to notify applicant and the department that he/she is or is not eligible for membership in the department according to the standards set forth by this pre-election exam. The results of this pre-election exam are to be kept on file by the facility and/or medical doctor, and are to be given out only to the person named on such exam as applicant. Such request shall be made in writing only. Should such a request be made and honored, the facility and/or medical doctor shall furnish this department with a copy of the written request and the date and time such copy was given to applicant. All costs for this pre-election exam are the sole responsibility of the department. It should be stressed to every applicant that the facility and/or medical doctor is hired by this department to perform the pre-election exam and is not a representative of this department.”
