Form 990, Page 6, Part VI, Line 12C

The organization requires all officers and board members to disclose conflicts of interest a least annually. In addition, they are encouraged to inform the executive director and board chair as soon as they perceive a conflict of interest. The executive director, board chair, and program director are responsible for reviewing potential conflicts of interst and determining whether a they consistute a conflict of interest that requires action. Typically, if a conflict is determined to exist, an the organization will hold an executive meeting to discuss the conflict and determine the action appropriate to mitigate the risks it poses to the organization.

The executive director's salary is determined by the board of directors. The executive director is not allowed to influence or vote at the board meetings. The board of directors review compensation given to executive directors at similar organizations in the region to determine a reasonable compensation level. Deliberation and determination of the executive director's salary is done during the annual board retreat. Approval is contemporaneously documented in the board minutes.

Form 990, Page 2, Part III, Line 4A

February 2024 hosted the 14th annual sweetheart challenge at utah tech university in st. George, with 25 attendees. The event focused on strengthening healthy relationships within the southern utah deaf community. March 2024 - presented at the utah school for the deaf to 15 high school students, providing information about sego lily's services and how to access support. - held a community education event on elder abuse at the salt lake center for the deaf and hard of hearing, with 15 attendees. - presented at utah tech university's psychology department to 35 students and a professor on supporting deaf survivors. A return invitation was extended for the fall. April 2024 - presented to the salt lake city sexual assault response team (sart) about trauma- informed support for deaf survivors. - delivered the empower deaf teens series to 10 middle school students at the utah school for the deaf, focusing on healthy relationships and boundaries. May 2024 - launched the annual may bloom social media campaign, raising 650 through educational posts from staff and board members. - held an in-person strategic planning weekend in st. George for board and staff to develop organizational goals. June 2024 - participated in utah pride and salt lake pride, hosting booths and selling fundraising items, raising 328.33. Distributed educational materials and resources. - presented at nevada coalition to end domestic and sexual violence's (ncedsv) virtual conference on serving deaf and hard of hearing survivors, with 30 participants in attendance. September 2024 - hosted the purple tie gala at the sanderson community center with a witch and wizard" theme. The event included auctions, entertainment, and community performances, raising approximately 23,362 with 225 attendees. October 2024 - received a rural grant from the department of justice, office on violence against women, to expand services in rural utah over the next three years. - partnered with deaflead to host a suicide prevention workshop, attended by 15 participants. - collaborated with the utah interpreting program and five star interpreting agency to provide ceu-eligible training titled empowering communication: trauma-informed practices for asl interpreters, with 25 interpreters and 15 community members. October-november 2024 - hosted the survivor's art show at deaf centers in salt lake city and st. George, displaying artwork that reflected healing from abuse. The impactful event will become an annual tradition. November 2024 - participated in the deaf bazaar at the sanderson community center with a fundraising booth. Featured a christmas tree with angel ornaments displaying survivors' holiday wishes. Community members donated gifts and funds to support families. December 2024 - held an end-of-year appreciation dinner with board and staff, celebrating the year's achievements and recognizing contributions to sego lily's mission.

OCTOBER 2024 - RECEIVED A RURAL GRANT FROM THE DEPARTMENT OF JUSTICE, OFFICE ON VIOLENCE AGAINST WOMEN, TO EXPAND SERVICES IN RURAL UTAH OVER THE NEXT THREE YEARS. - PARTNERED WITH DEAFLEAD TO HOST A SUICIDE PREVENTION WORKSHOP, ATTENDED BY 15 PARTICIPANTS. - COLLABORATED WITH THE UTAH INTERPRETING PROGRAM AND FIVE STAR INTERPRETING AGENCY TO PROVIDE CEU-ELIGIBLE TRAINING TITLED EMPOWERING COMMUNICATION: TRAUMA-INFORMED PRACTICES FOR ASL INTERPRETERS, WITH 25 INTERPRETERS AND 15 COMMUNITY MEMBERS. OCTOBER-NOVEMBER 2024 - HOSTED THE SURVIVOR'S ART SHOW AT DEAF CENTERS IN SALT LAKE CITY AND ST. GEORGE, DISPLAYING ARTWORK THAT REFLECTED HEALING FROM ABUSE. THE IMPACTFUL EVENT WILL BECOME AN ANNUAL TRADITION. NOVEMBER 2024 - PARTICIPATED IN THE DEAF BAZAAR AT THE SANDERSON COMMUNITY CENTER WITH A FUNDRAISING BOOTH. FEATURED A CHRISTMAS TREE WITH ANGEL ORNAMENTS DISPLAYING SURVIVORS' HOLIDAY WISHES. COMMUNITY MEMBERS DONATED GIFTS AND FUNDS TO SUPPORT FAMILIES.
