Accomplishments: health council grant accomplishments: -re-established a youth health council at the santa rosa high school with 15 members - the youth participated in the youth mental health first aid training, gave presentations on health issues at the middle and high school. -one health council member was trained in qpr a suicide prevention program as a training obtaining for the community 2 trainers that will provide trainings in guadalupe county. -988 campaigns throughout the year to promote the state hotline. -held a successful health fair in collaboration with guadalupe county hospital. -held the 2nd annual art gala for recovery month in collaboration with local collaborative 4. -continued to participate in the local collaborative 4 to address behavioral health needs in the county. Healthy kids healthy communities grant accomplishments: -partnered with the anton chico school to establish a farmers market, worked with a community member to attend the statewide meeting and assisted with paperwork for the market. -santa rosa elementary garden has been established with 2 school champions. -all senior centers and the santa rosa elementary schools received either seed or sprout level awards for the golden chile awards for their participation in the nm grown program to promote healthy meals and eating locally grown foods. -eat smart to play hard and 521o challenges completed in all three elementary schools. -walk and roll to school events two in october at the santa rosa and vaughn elementary schools, and one in may at the anton chico school. Students either walked as a group or rode their bikes to school on those two days. -collaborated with q creations to apply for a trails+ grant to plan a mountain bike path in santa rosa, the grant was awarded to q creations in the amount of 99,999.

IRS990ScheduleO/SupplementalInformationDetail/ExplanationTxt6ACCOMPLISHMENTS: HEALTH COUNCIL GRANT ACCOMPLISHMENTS: -RE-ESTABLISHED A YOUTH HEALTH COUNCIL AT THE SANTA ROSA HIGH SCHOOL WITH 15 MEMBERS - THE YOUTH PARTICIPATED IN THE YOUTH MENTAL HEALTH FIRST AID TRAINING, GAVE PRESENTATIONS ON HEALTH ISSUES AT THE MIDDLE AND HIGH SCHOOL. -ONE HEALTH COUNCIL MEMBER WAS TRAINED IN QPR A SUICIDE PREVENTION PROGRAM AS A TRAINING OBTAINING FOR THE COMMUNITY 2 TRAINERS THAT WILL PROVIDE TRAININGS IN GUADALUPE COUNTY. -988 CAMPAIGNS THROUGHOUT THE YEAR TO PROMOTE THE STATE HOTLINE. -HELD A SUCCESSFUL HEALTH FAIR IN COLLABORATION WITH GUADALUPE COUNTY HOSPITAL. -HELD THE 2ND ANNUAL ART GALA FOR RECOVERY MONTH IN COLLABORATION WITH LOCAL COLLABORATIVE 4. -CONTINUED TO PARTICIPATE IN THE LOCAL COLLABORATIVE 4 TO ADDRESS BEHAVIORAL HEALTH NEEDS IN THE COUNTY. HEALTHY KIDS HEALTHY COMMUNITIES GRANT ACCOMPLISHMENTS: -PARTNERED WITH THE ANTON CHICO SCHOOL TO ESTABLISH A FARMERS MARKET, WORKED WITH A COMMUNITY MEMBER TO ATTEND THE STATEWIDE MEETING AND ASSISTED WITH PAPERWORK FOR THE MARKET. -SANTA ROSA ELEMENTARY GARDEN HAS BEEN ESTABLISHED WITH 2 SCHOOL CHAMPIONS. -ALL SENIOR CENTERS AND THE SANTA ROSA ELEMENTARY SCHOOLS RECEIVED EITHER SEED OR SPROUT LEVEL AWARDS FOR THE GOLDEN CHILE AWARDS FOR THEIR PARTICIPATION IN THE NM GROWN PROGRAM TO PROMOTE HEALTHY MEALS AND EATING LOCALLY GROWN FOODS. -EAT SMART TO PLAY HARD AND 521O CHALLENGES COMPLETED IN ALL THREE ELEMENTARY SCHOOLS. -WALK AND ROLL TO SCHOOL EVENTS TWO IN OCTOBER AT THE SANTA ROSA AND VAUGHN ELEMENTARY SCHOOLS, AND ONE IN MAY AT THE ANTON CHICO SCHOOL. STUDENTS EITHER WALKED AS A GROUP OR RODE THEIR BIKES TO SCHOOL ON THOSE TWO DAYS. -COLLABORATED WITH Q CREATIONS TO APPLY FOR A TRAILS+ GRANT TO PLAN A MOUNTAIN BIKE PATH IN SANTA ROSA, THE GRANT WAS AWARDED TO Q CREATIONS IN THE AMOUNT OF 99,999.
