The form was prepared by Tax Specialists of the Carolinas based on the financials provided by Michael Ambrose, and a draft was distributed to the director. Following this review, the board records the approval of the Form 990 in the meeting minutes. This documentation confirms that the form was reviewed and approved prior to final submission. The Form 990 is subsequently filed with the IRS.
