“All board members receive a full electronic copy of the Form 990 and are asked to review and provide comments or questions. The Executive Committee, made up of board members, received an electronic copy in advance of an Executive Committee meeting at which the Form 990 was reviewed and discussed.”

“The conflict of interest policy was adopted by the Board of Directors just before the organization's year end on June 28, 2021. It was discussed at a Board meeting and all board members were asked to sign an annual statement of compliance. The Governance Committee is charged with handling potential conflicts of interest and monitors issues as they arise.”

“Governing documents and policies are available upon request. They will be emailed or mailed in paper form if requested. The organization has not maintained regular financial statements to date but will make them available upon request as they are prepared.”

“We are correcting our schedule of disqualified persons to include additional donations not originally reported from certain board members and schedule B to correct reporting of donations from Donor Advised funds and certain company matching donations.”

“This was our first year. Created and disseminated sugary drinks poster to pediatrician offices to raise awareness of pediatric fatty liver disease, sponsored virtual "Liver Healing with HeART" program for pediatric liver patients. We began planning a monthly, free, virtual webinar series "Conversations with Liver Experts" that did not start until September 2021.”
