Conflict of interest policy compliance Part VI line 12C

The conflict of interest policy requires each director, principal officer and committee member to sign a statement every year. This statement affirms that these persons have received a copy of the policy, understand and agree to comply with the policy. The procedures require that any interested person must disclose their financial interest, and they shall be excused from discussion and voting on the matter. The minutes of the meeting shall disclose all actions with regard to the transaction.

CHS provides a patient assistance program for low-income patients: however, the organizations regular fees are nearly 60% less than the average charged in hospital-owned facilities.
