“Susan's legacy is non-profit organization committed to serving women with co-occuring mental and addictive disorders(cod). The services include intensive case management, psyco-education groups, including 12-step empowerment recovery groups,life skills education and substance abuse assessment and counseling. The goal of this program is to grow participant's skills as they become independent, self-supporting, contributting members of the commmunity.”

“Susan's legacy provided intensive case management during the year. All clients were connected with the community resources they needed, like furniture, clothing, ect. Those clients not already enrolled in medicaid or food stamps were enrolled. Susan's legacy assisted clients with finding housing. If not already connected with a primary care physician and/or psychiatrist, clients were assisted with as needed, many clients were connected with resources for their children. The intensive case manager met with or called clients on a weekly basis. The organization provided mental health counseling for all clients on a weekly basis and provided a weekly addiction support group as well as weekly or bi-weekly individual addition counseling sessions for all clients.”
