Governance Explanations

Form 990, Part VI, Section B, Line 11B

The 990 is prepared by the external auditor. A draft, along with pending questions, is presented to the executive director and in-house accountant for review and completion of unanswered questions. Once it is reviewed and complete, it is reviewed by the finance committee before filing. A copy is provided to each member of the board of director.

Form 990, Part VI, Section B, Line 12C

All staff, volunteers, and board members are required to sign a conflict of interest policy statement. Board members are asked to disclose any direct or indirect conflict of interest situations upon joining the board and must confirm this information annually. Employees are governed by the same policy, which is reiterated in the organization's personnel policies and procedures.

Form 990, Part VI, Section C, Line 19

Governing documents, conflict of interest policy, and financial statements are made available upon request.
