The Form 990 is prepared by an independent accounting firm based on the financial statements and information provided to them by the accounting department at CCHS, a related organization. After the Form 990 is prepared, it is reviewed by the Controller and CFO of CCHS. It is also provided to the Board of Directors for review prior to filing with the IRS.

Form 990, Part VI, Section B, line 12C

Once a year, Board members are required to certify that they have read the conflict of interest policy and disclose any relationships which might have the appearance of a conflict of interest. This is reviewed by the Service and Quality Improvement Manager. Board members who are determined to have a conflict of interest will abstain from participating in discussion or voting on the matter related to that conflict.

As a public charity, the entity is exempt from state and federal income taxes on income earned in accordance with their tax-exempt purpose. Unrelated business income is subject to state and federal income tax. Management has evaluated the Organization's tax position and concluded that the Organization has no unrelated business income or uncertain tax positions that require adjustment to the consolidated financial statements.
