The archdiocese of st. Louis (the archdiocese) periodically provides the organization with certain management services. The archdiocese pays all expenses, salaries, and fringe benefits including payroll, for the organization. In addition, the federal w-2 forms for salaries and payroll tax returns are included on the archdiocese' records. The organization reimburses the archdiocese on a regular basis for these project expenditures and there is no payment to the managing agent for these services. Rural parish clinic had 2 employees during the year ended june 30, 2021

The organization was incorported to allow a "member" which named the archbishop of the archdiocese of st. Louis to retain governance authority on a discretional basis.

The return is prepared by an independent accounting firm. The process for 990 review will include distribution by the executive director of the 990 final version to the board members to allow review prior to the filing of the return. An opportunity for any questions or request for amendment will be honored and complied with prior to filing.

There are two compensated directors in the rural parish clinic. The medical director received a limited stipend which is negotiated between the religious order to which the medical director is member of and the archdiocese, outside the operations of the rural parish clinic. The executive director / administrator is compensated by the rural parish clinic. The initial compensation was determined by input from the catholic charities leadership and the medical director with input from the executive director. At the end of october 2021, the executive director resigned and restructuring of the corporation was initiated under the direction of the board of directors to include a medical clinic administrator and a dental clinic administrator in place of an executive director to oversee the management of each clinic. These positions have not yet been filled. All employees of rural parish clinic are paid by the archdiocese of st. Louis (the archdiocese) . The archdiocese uses a salary administration program, including pay grades and ranges, that is provided by catholic charities of st. Louis. Additionally, the st. Louis area salary survey report issued annually by the aaim management association is reviewed when preparing salary budgets for the fiscal year. The organization also reviews national and local inflation rates, internal funding abilities, and planned salary budgets for the archdiocese of st. Louis. Annual performance evaluations are also reviewed when determining salary increases.

No information has been shared to date regarding governing documents, conflict of interest policy or financial statements to the public. Once completed, only the 990 would be considered to be shared with the public by filing the 990 with the irs. Any inquiry or request would be received or forwarded to the executive director who would respond and provide the appropriate documents in a timely manner. All other board documents requested to be copied would be reviewed with legal counsel and possibly the board for a decision to share in part or whole or declined to the requesting party.

The individual agencies that comprise the archdiocese are listed in the official catholic directory and, therefore, are tax-exempt public charities under section 501(c)(3) and section 509(a) of the internal revenue code, except for holy infant & st. Joseph associates, lp, rosati apartments, lp and st. John neumann associates, lp. Holy infant & st. Joseph associates, lp, rosati apartments, lp and st. John neumann associates, lp, are partnerships established as passthrough entities for tax purposes. As such, the archdiocese can only be taxed on income from any activities unrelated to their charitable purpose. At june 30, 2021 and 2020, the archdiocese had not identified any such revenue; therefore, no tax expense has been recorded. The archdiocese does not have any uncertain tax positions.

THE MOBILE DENTAL CLINIC ESTABLISHED ITS THIRD SITE IN OCTOBER 2021, AND HAS ALREDY SEEN NEARLY 200 PATIENTS IN ITS FIRST YEAR. PATIENTS WITHOUT INSURANCE AND INCOME <200% FEDERAL POVERTY LEVEL CAN CALL FOR SERVICES ON EITHER OR BOTH VANS WITHOUT COST. THE MEDICAL VAN PROVIDES PRIMARY HEALTH CARE, HEALTH MAINTENANCE, LABORATORY SERVICES AND SOME MEDICATIONS FREE OF CHARGE. THE MOBILE DENTAL UNIT PROVIDES PRIMARY DENTAL CARE AND SOME HYGIENE AS WELL AS EXTRACTIONS, PARTIAL AND FULL DENTURES AT NO COST TO PATIENTS. THE RURAL PARISH CLINIC TRACKS THE SOCIAL DETERMINANTS OF HEALTH FOR ITS PATIENTS AND REPORTS THAT 90% ARE BELOW THE 100% FEDERAL POVERTY GUIDELINES, 18% OF PATIENTS AVOIDED AN ER VISIT BY COMING TO RPC AS A LAST RESORT DUE TO INDIGENCY, 60% WERE UNEMPLOYED, 7% REPORTED HOMELESS, 26% FOOD INSECURE, 8% NEEDING TRANSPORTATION ASSISTANCE AND 14% NEEDING UTILITY BILL ASSISTANCE.

BUDGETS WERE CREATED AND ALLOCATION OF INITIAL DENTAL EXPENSES ESTIMATED AND WERE SET UP APART FROM MEDICAL OPERATIONS. ADDITIONAL ALLOCATION OF ANY GRANTS, DONATIONS OR OTHER FUNDING FOR DENTAL SERVICES WERE ALSO SEPARATED FROM MEDICAL. THE DENTAL VAN CONSTRUCTION WAS COMPLETED AND DELIVERED IN SEPTEMBER OF 2020. THE MISSION STARTED SEEING PATIENTS IN JANUARY 2021, STAFFED BY A DENTAL COORDINATOR AND SERVICES FULLY BY VOLUNTEER PROVIDERS.

THE INITIAL COMPENSATION WAS DETERMINED BY INPUT FROM THE CATHOLIC CHARITIES LEADERSHIP AND THE MEDICAL DIRECTOR WITH INPUT FROM THE EXECUTIVE DIRECTOR.
