The health coverage guides program began in august 2013 through collaboration with the nw colorado visiting nurses association. Health coverage guides help residents of grand and jackson counties enroll in the colorado's health insurance marketplace and medicaid. In 2013, the connect for health data indicated that 215 grand county residents and 16 jackson county residents enrolled in the marketplace. Additionally, 173 grand county residents enrolled in medicaid through the markeplace.

The partners for adults in need of services (p.a.i.n.s.) program offers healthcare vouchers for uninsured, low-income adults ages 18 to 65 living in grand county. In 2013, the p.a.i.n.s. Program issued 132 vouchers to 117 unique clients. 26,562 cancer-related services began in 2012 to assist grand county residents with any type of cancer. This program is largely supported by volunteer fundraising. The cancer-related services program issues vouchers for gas, hotels, and prescription assistance to residents receiving cancer screenings, treatment or follow-up. Our goal is to remove transportation, lodging and financial status as barriers to cancer care and screening. In 2013, the program issued 78 vouchers to 25 unique clients. 23,242 advocacy and the healthy grand county 2020 plan focuses on improving the health of grand county by the year 2020. The plan concentrates on the winnable topic areas, such as the meeting milestones initiative, which implements standardized developmental screenings in healthcare and childcare settings while working toward an online platform to collect child- and aggregate-level data. This also includes an all-inclusive society of healthcare professionals from all sectors who collaborate to put the patient first. The society met 6 times with approximately 15 regular members. Staff also attended 32 community education events and participated in 5 advisory committees to improve community health. 16,601

PROGRAM ISSUED 132 VOUCHERS TO 117 UNIQUE CLIENTS. 26,562 CANCER-RELATED SERVICES BEGAN IN 2012 TO ASSIST GRAND COUNTY RESIDENTS WITH ANY TYPE OF CANCER. THIS PROGRAM IS LARGELY SUPPORTED BY VOLUNTEER FUNDRAISING. THE CANCER-RELATED SERVICES PROGRAM ISSUES VOUCHERS FOR GAS, HOTELS, AND PRESCRIPTION ASSISTANCE TO RESIDENTS RECEIVING CANCER SCREENINGS, TREATMENT OR FOLLOW-UP. OUR GOAL IS TO REMOVE TRANSPORTATION, LODGING AND FINANCIAL STATUS AS BARRIERS TO CANCER CARE AND SCREENING. THE PLAN CONCENTRATES ON THE WINNABLE TOPIC AREAS, SUCH AS THE MEETING MILESTONES INITIATIVE, WHICH IMPLEMENTS STANDARDIZED DEVELOPMENTAL SCREENINGS IN HEALTHCARE AND CHILDCARE SETTINGS WHILE WORKING TOWARD AN ONLINE PLATFORM TO COLLECT CHILD- AND AGGREGATE-LEVEL DATA.
