Board review of form 990: the board of directors reviews the form 990 with the return preparer at a board meeting prior to filing the return with the irs.

Conflict of interest policy: each board member reviews and signs the conflict of interest policy when they join the board of directors. During fiscal year 6/30/2014, the conflict of interest policy was reviewed with board members to ensure compliance.

Compensation setting process: the executive director is hired through a board of directors committee and if necessary, aided by a recruiting firm. Comparability data is used. Salary is negotiated between the executive director and the committee. Deliberation and conclusion of compensation process is documented in the board minutes. The executive director's compensation is also documented in a written contract. Key employees are hired through a multiple interview process made up of agency wide employees and external individuals, using comparability data. The final interview is done by the executive director and salary is negotiated between the executive director and the key employee. Compensation is documented in the employee's personnel file. The compensation for the cfo position is reviewed and approved by the board of directors.

The executive committee shall be composed of the officers of the corporation and two additional board members who are elected by the board at the annual meeting of the board of directors. The executive committee, as delegated to it by the board, shall have, and may excercise, the powers of the board as the need arises between meetings of the board. The executive committee shall also be responsible for coordinating and administering personnel matters relating to the executive director, including the administration of the executive director's annual performance review. The executive/personnel committee shall take the lead in hiring or replacment of the executive director; however, in the latter case, the executive committee may make recommendations only and may not act unilaterally without the approval of the full board of directors.

Part III, Line 4D

*laradon became an employment network and can provide employment services to adults who receive social security disability insurance (ssdi). *laradon expanded services to senior citizens and are now serving adults with alzheimer's disease and dementia. *laradon's family, infant and toddler (fit) program increased capacity by offering individualized training to parents and siblings so that therapeutic activities could take place for their infant throughout the week in addition to the weekly therapy sessions. As a result, infants and toddlers are reaching their developmental milestones on time. *laradon's participants in the community participation programs volunteered at a variety of other nonprofit agencies in our community. The result was over 6000 hours of service was provided. *laradon expanded the number of metro area school districts that refer students to the alternative school to 17, which means that more students are getting the services they need to overcome their challenges and become independent adults. *laradon has started using computerized data acquisition and management systems which have resulted in less paperwork and more increase in efficiency.

Fin 48 footnote laradon applies a more-likely-than-not measurement methodology to reflect the financial statement impact of tax positions taken or expected to be taken in a tax return. After evaluating the tax positions taken, none are considered to be uncertain; therefore, no amounts have been recognized as of june 30, 2014 or 2013. If incurred, interest and penalties associated with tax positions are recorded in the period assessed as general and administrative expenses. No interest or penalties have been assessed as of june 30, 2014 or 2013. Tax years that remain subject to examination include 2011 through the current year for the federal return and 2010 through the current year for the colorado return.

TODDLERS GRADUATE OR "AGE OUT" OF THE PROGRAM ON THEIR THIRD BIRTHDAY. (CONTINUED ON SCHEDULE O) 2. THE ALTERNATIVE SCHOOL PROVIDES INDIVIDUALIZED EDUCATION TO STUDENTS 5 TO 21 YEARS OF AGE WHO ARE DUALLY DIAGNOSED WITH BOTH A DEVELOPMENTAL DISABILITY AND A BEHAVIORAL, EMOTIONAL OR PSYCHIATRIC DISORDER. REFERRALS TO THE SCHOOL ARE MADE THROUGH LOCAL SCHOOL DISTRICTS. THE ALTERNATIVE SCHOOL'S CURRICULUM UTILIZES A FUNCTIONAL APPROACH TO TEACHING ACADEMICS, LIFE AND VOCATIONAL SKILLS, AS WELL AS PROVIDING THE MENTAL HEALTH SUPPORT THE STUDENTS REQUIRE. STUDENTS RECEIVE AFFECTIVE EDUCATION AND HAVE INDIVIDUALIZED POSITIVE BEHAVIOR INTERVENTION PROGRAMS BASED ON THEIR UNIQUE NEEDS.
