Form 990 governing body review Part VI line 11

The Organization has its form 990 prepared by an outside accounting firm and has established the following review process to ensure that the information reportedis complete and accurate. When the form 990 has been prepared, reviewed by management and is ready to be filed with the Internal Revenue Service, it is electronically sent to the board for approval. Once the board has approved the return it is electronically filed with the Internal Revenue Service.

Conflict of interest policy compliance Part VI line 12C

The Organization curently has in place a conflict of interest policy that applies to any employee, executive director, and member of the board of directors. The Organization regularly and consistently monitors and enforces compliance with the conflict of interest policy. They refrain from and avoid potential conflicts of interest, or the appearance of conflicts, between the private interests of any employee and independent contractors and his or her official responsibilities and duties performed on behalf of the organization.

The board of directors approves and reviews the compensation of the executive director. The process for determining executive compensation consists of the board pulling comparability data(such as form 990 of similar sizes organization) and then reviewing the date, discussing key considerations, including the number of employees being managed, revenue, work load, and years of experience, and then voting to approve an appropriate salary. This process was last undertaken in 2019 and the records of the compensation decisions are documented in the board meeting minutes.
